无柄人工髋关节置换术的临床研究

摘  要:目的:探讨无柄人工髋关节置换术的治疗效果。方法:取患髋后外侧切口显露髋关节,股骨头脱出髋臼,清除髋关节内与周围病变组织,打入中心定位器针,并用塑骨器成型,安装罩体,固定中心钉和大、小粗隆松质骨螺钉。髋关节置换术同样方法安装髋臼假体。结果:本组30例,平均随访1年8个月。按Harris关节功能评价标准评估,优17例,良6例,优良率为76.67%;松动6例,占20.00%,经再次手术优5例,调整后优良率为93.1%;感染1例,占3.45%.结论:无柄人工髋关节置换术,由于保留了全部的股骨颈和部分股骨头,力学传导与分布更接近于人体的自然生物力学状态,远期疗效仍有待进一步观察。Objective: To detect the curative effect of the sessile artificial hip prosthetic replacement. Methods: Exposed the hip joint through posterior-exterior incision, dislocated the femoral head, cleared away the sick tissues inside joint, throw into the centric localizer, repaired bone by the model machine, set the cup and fixed cancellous bone screw into major and minor troehanter of the femoral. The traditional method was also applied to fix the acetabulum prosthesis. Results: Thirty cases were followed up for 20 months averagely in the group. There were 17 excellent cases and 6 good cases, with excel- lence and good rate of 76.67%. There were 6 cases of loose (20%). Five cases were excellence after the second operation, and the adjustment excellence rate was 93.1%. One case was infection (3.45%). Conclusion:The mechanical transmission and distribution is more close to the body's natural biological mechanics state in the sessile artificial hip prosthetic replacement for it retained the femoral neck and part of the femoral head, but its long term curative effect remains to be further observation.
