摘 要:目的:通过分析听力图正常耳鸣患者听性脑干反应(ABR)的特点检测可能存在的隐匿性听力损失,探讨耳鸣的起源。方法:对2010-2012年因单纯性耳鸣就诊于我院的40例患者与15例正常对照者进行纯音听阈、ABR听阈、Ⅰ波及Ⅴ波波幅的检测,并分析其结果。结果:耳鸣患者纯音听阈及ABR阈值与正常对照者无明显差异,但耳鸣患者ABRⅠ波波幅下降,Ⅴ波波幅则恢复正常水平。结论:听力图正常的耳鸣患者在初级听神经水平存在隐匿性听力损失,脑干水平的稳态维持机制可能是产生耳鸣的基础。Objective:To investigate hidden hearing loss in tinnitus patients with normal audiograms by means of auditory brainstem response(ABR) and explore the origin of tinnitus.Method:Pure tone thresholds,ABR thresholds,amplitude of wave Ⅰ and wave Ⅴ of ABR were analyzed in 40 tinnitus patients and 15 controls.Result:There was no significantly difference in pure tone thresholds and ABR thresholds between those tinnitus patients and controls while a reduced amplitude of wave Ⅰ and normal amplitude of wave Ⅴ of ABR in the tinnitus patients became evident.Conclusion:Tinnitus patients with normal audiograms have hidden hearing loss at the level of primary auditory nerve and the generation of tinnitus is likely attributed to a homeostatic response of neurons in brainstem.
