自身抗体在乙型和丙型肝炎及自身免疫性肝炎中的特点分析

摘　　要：目的:分析自身抗体在乙型肝炎、丙型肝炎及自身免疫性肝炎中(AIH)的特点。方法:将患者分为4组,HBV组60例,HCV组60例,AIH组34例,健康对照组60例,采用间接免疫荧光法和欧蒙斑点法检测血清中的ANA、AMA-2、LKM-1、LC-1、SLA/LP、SMA等自身抗体。结果:1)HBV、HCV、AIH组ANA检出率均明显高于健康对照组,AIH组ANA阳性率显著高于HBV和HCV组(P<0.05)。2)HBV、HCV组ANA以低滴度(1:100)为主,AIH组ANA以高滴度(≥1∶320)为主。3)AIH组SMA、AMA-M2、SLA/LP、LKM-1、LC-1阳性率分别为55.9%、2.9%、11.8%、17.6%、2.9%,HBV组AMA-M2阳性1例,HCV组LKM-1阳性2例。结论:乙型、丙型肝炎患者血清中存在自身抗体,但滴度较低,自身免疫性肝炎患者具有高滴度的自身抗体水平;检测自身抗体对于肝脏疾病的诊断及治疗具有重要作用。Objective ：To analyze the characteristics of autoantibodies in hepatitis B, hepatitis C and autoimmune hepatitis （AIH）. Methods： The patients were divided into four groups： HBV group （60 cases）, HCV group （60 cases）, AIH group （34 cases）, and healthy adults （60 cases） as control. Indirect immunofluorescence assay and EUROASSAY were used to detect the serum ANA, AMA - 2, the LKM - 1, LC - 1, SLA/LP, SMA. Results： 1. The detection rates of ANA in HBV, HCV and AIH group were significantly higher than healthy controls, and the positive rate of ANA in AIH group was significantly higher than that in HBV and HCV group （P 〈 0. 05）. 2. HBV and HCV group had low titer（1： 100） of ANA, while AIH group had high titer （ ≥1： 320） of ANA. 3. In AIH group, the positive rates of SAM, AMA- M2, SLA/LP, LKM- 1, LC- 1 were 55.9%, 2.9%, 11.8%, 17.6% and 2.9%. One case in HBV group was detected with positive AMA - M2, and 2 cases in HCV group were detected with positive LKM - 1. Conclusion： Patients with HBV or HCV infection had＇ autoantibodies with low titers, while patients with AIH had high titers of autoantibodies; The detection of autoantibodies play an important role in the di- agnosis and treatment of liver diseases.
