摘  要:目的探讨新生儿行为神经测定(NBNA)评分法和加分法在足月高危儿脑损伤筛查及早期干预的应用价值。方法对396例足月高危儿分别于出生后2~3d、5~7d、12~16d、26~28d进行NBNA基础项目评分及加分项评分,对评分≤35分者早期干预,包括视、听、触觉等方面的刺激;婴儿期采用0~6岁小儿神经心理发育检测法测查并予分析。结果新生儿期NBNA评分≤35分者,经早期干预,生后12~16d组、生后26~28d组与生后5~7d组比较差异显著,随干预进行后缺氧缺血性脑病(HIE)患儿所占比例显著减少;NBNA加分测试法显示随着疾病恢复、自身发育及早期干预,加分值呈上升趋势,尤其HIE患儿;随访至12个月时,发育商比较差异有非常显著性意义。结论应用NBNA评分法对高危儿脑损伤早期筛查、早期干预具有重要应用价值,早期干预可有效降低脑损伤后遗症发生。Objective: To explore newborn behavior neural measurement (NBNA) score method and add cent method in term high - risk brain damage screening and early intervention application value. Methods : 396 cases of full live undergraduate high - risk re- spectively after birth in 2 - 3 d, 5 - 7 d, 12 - 16 d, 26 - 28 d to NBNA score and a basic project for score, to score 35 points or less the early intervention, including sight and hearing and touch of stimulus ; Infantile using 0 - 6 years old children nerve psycho- logical development assay examination and to analysis. Results : The neonatal period NBNA score 35 points or less, are the early intervention, born after 12 - 16 d group, born after 26 - 28 d group after birth and 5 - 7 d group significant difference compared with intervention after hypoxia ischemic encephalopathy (HIE) the proportion of patients significantly reduced ; NBNA points test shows that as the disease recovery, their own development and early intervention, add score shows ascendant trend, especially children HIE, Fol- low- up to 12 months, developmental quotient comparative differences have very significant meaning. Conclusion: Application of NB- NA score method to high- risk brain injury early screening, early intervention has important application value, early intervention can effectively reduce the brain damage occurred sequela.
