摘 要:目的探讨在医院船内对海水浸泡大批医疗器械的清洗效果,提高海上灾害的救护能力。方法根据实验地点不同,本实验分为陆战伤组(在码头实验室),海上组(在医院船上,海上风力4~6级;海面浪高3~5 m,摇摆度在15°~30°),每组各类器械共800件。海上组器械行海水脏腔冲洗液(动物血液、脏腔液、海水和生理盐水)浸泡4 h;陆地组器械行脏腔冲洗液(动物血液、脏腔液、和生理盐水)浸泡4 h。器械清洗消毒采用全自动清洗消毒机。实验监测采用肉眼与镜下观察、隐血检测和细菌培养检测,两组的合格率进行比较。结果陆地组肉眼与镜下检测、隐血试验、细菌培养合格率分别为67%,62%,66%和96%;海上组分别为49%,41%,47%和75%。海上组各项指标均明显低于陆地组(P<0.01)。结论海上环境对海水浸泡大批医疗器械清洗质量有明显的影响。掌握医院船内器械清洗特点,提高供应室人员海上的操作技巧和对海上环境的适应能力是提高器械清洗质量的关键。Objective To explore the cleaning efficacy of large numbers of medical instruments with seawater immersion in hospital ship so as to improve the medical rescue capacity for maritime disasters. Methods According to different experiment places,the experiments were divided into land group(on dock)and sea group(wind force 4-6,wave height 3-5 m,rock degree 15°-30° in hospital ship) with 800 pieces of medical instruments in each group.The instruments were polluted with wash liquid of seawater splanchnocoel(animal blood,splanchnocoel liquid,seawater,and wash liquid of normal saline) for 4 hours in sea group.And the instruments were polluted with wash liquid of splanchnocoel(animal blood,splanchnocoel liquid and wash liquid of normal saline) for 4 hours in land group.The instruments were washed with full automatic cleaning-disinfecting machine.Naked eye and microscope observation,occult blood test and germiculture were used to examine the efficacy of the clean method.Qualification rates of the two groups were compared. Results Qualification rates were 67%,62%,66% and 96% in land group and 49%,41%,47% and 75% in sea group by naked eye and microscope observation,occult blood test and germiculture test.All indexes were apparently lower in sea group(P0.01). Conclusion The environment on sea has obvious affection on the cleaning quality of large numbers of medical instruments with seawater immersion.Grasping cleaning characteristics,improving the quality of the operations skill on thesea and adaptability to the environment on sea of the staff in the disinfection room are crucial to increase the cleaning quality.
