改良逆行气管插管在呼吸衰竭中的综合应用价值研究

摘　　要：目的探讨改良逆行气管插管在呼吸衰竭中的综合应用价值。方法随机选取2011年3月-2012年8月于广东医学院附属石龙博爱医院采用改良逆行气管插管法进行抢救的30例呼吸衰竭伴困难气道患者为观察组,同期采用传统逆行气管插管法进行抢救的30例患者为对照组,后将两组患者的插管时间、第1,2,3次插管成功率、并发症发生率、插管前及插管中的血流动力学指标进行比较。结果观察组的插管时间短于对照组,第1次插管成功率高于对照组,并发症发生率低于对照组,插管中的SBP、DBP、HR及MAP均低于对照组,P均<0.05,差异均有统计学意义。结论改良逆行气管插管在呼吸衰竭中的综合应用价值较高,对患者的不良刺激更小。Objective To study the application value of modified retrograde tracheal intubation in patients with respiratory failure. Methods Total 30 patients with respiratory failure and difficult airway who were rescued by modified retrograde tracheal intubation in our hospital from March 2011 to August 2012 were selected as observation group, and 30 patients who were rescued by traditional retrograde tracheal intubation at the same time were selected as control group, then the intubation time, success rate of intubation at first, second and third time, incidence of complications, hemodynamics indexes before and during intubation of two groups were compared. Results The intubation time of observation group was shorter than those of control group, success rate of intubation at first time was higher than those of control group, incidence of complications was lower than those of control group, SBP, DBP, HR and MAP during intubation were all lower than those of control group, all P 〈 0.05, there were all significant differences. Conclusion The comprehensive application value of modified retrograde tracheal intubation in patients with respiratory failure is higher,and the bad stimulation for the patients is smaller.
