心血管患者外周静脉留置针留置时间与细菌定植研究

摘　　要：目的从细菌学的角度探讨心血管病患者外周静脉留置针各个留置部位的最佳留置时间。方法实验随机抽取2009年11月～2010年7月该院心血管内科收治的240例住院患者,根据留置针留置部位和留置时间将患者分为3组、每组4个时间段,分别对患者穿刺部位的皮肤(皮肤消毒后、拔针时)和留置针导管采样后进行细菌培养与鉴定。对实验结果采用SPSS 13.0统计软件进行分析。结果①皮肤消毒后,穿刺部位皮肤样本细菌培养均为无菌生长。②3组患者拔针时穿刺部位皮肤样本细菌培养,41例有菌生长;3组患者T(148h≤t<72 h)、T(272 h≤t≤96 h)均未检出细菌,4个时间段比较,P<0.05,差异有显著性。③3组患者拔针时留置针导管样本细菌培养,10例有菌生长;手背组、前臂组两组患者4个时间段均未检出细菌,足背组患者T(148h≤t<72 h)、T(272 h≤t≤96 h)未检出细菌,T(396 h<t≤120 h)检出4例、T4(120 h<t≤168 h)检出6例,4个时间段比较,P<0.05,差异有显著性。结论心内科患者留置在手背和足背的留置针留置时间不宜超过96 h,留置在前臂的留置针留置时间不宜超过120 h。【Objectives】 To investigate the best indwelling time at different insertion sites of peripheral intravenous indwelling catheter in cardiovascular disease patients.【Methods】 240 hospitalized patients in this study were randomly recruited in cardiovascular department of Xiang-Ya Hospital of Central South University from Sep 2009 to Jul 2010.All subjects were divided into 3 groups according to the insertion site of peripheral intravenous indwelling catheter： forearms（group 1）,back of hands（group 2）,feet（group 3）.After obtaining specimens at skin puncture site respectively after skin disinfection and extracting catheters,as well as on the extracted catheters,culture and identification of bacteria were performed.Multiple measurements were performed at 4 different times for each group.All calculations were performed using SPSS V13.0 for Windows.【Results】 ①After skin disinfection,bacterial culture of these three groups showed to be sterile.②After extracting catheters,culture of bacteria showed 41 cases with bacterial growth;no bacteria growth was detected among three groups at T1（48 h≤t72 h） and T2（72 h≤t≤96 h）;Comparison in three groups were considered statistically significant（P 0.05） at 4 different times.③Specimens from the extracted catheters were cultured and results showed 10 cases with bacterial growth;no bacterial growth was assessed in group 1 and group 2 at 4 different times;bacterial culture of group 3 showed to be sterile at T1（48 h≤t72 h） and T2（72 h≤t≤96 h）,4 cases at T3（96 ht≤120 h） and 6 cases at T4（120 h﹤t≤168 h）with bacterial growth.Comparison in group 3 was considered statistically significant（P 0.05） at 4 different times.【Conclusion】 The duration of indwelling on the back of hands or feet with peripheral intravenous catheter should be no more than 96 hours,while on the forearms it should be no more than 120 hours.
