摘  要:目的探讨前后联合入路治疗髋臼骨折并中心性脱位的疗效。方法对2008年6月~2011年12月采用前后联合入路治疗的24例髋臼骨折并中心性脱位进行回顾性分析。均采用切开复位重建钢板螺钉内固定术。结果所有患者获得6~36个月(平均15个月)随访。临床结果按照Matta标准:解剖复位15例,满意复位9例。髋关节功能采用Matta改良的d'Aubigne-Postel评分标准评定:优7例,良14例,可3例,优良率87.5%。创伤性关节炎6例,异位骨化4例。医源性股外侧皮神经损伤3例,均为不完全性损伤,术后1个月左右完全恢复。结论手术治疗髋臼骨折并中心性脱位时,采用前后联合入路显露较好,方便骨折复位及固定,并发症较少,可获得满意的临床疗效。Objective To study the efficacy of surgery treatment of acetabular fracture with central dislocation of the hip joint with anterior and posterior approaches. Methods A retrospective analysis was conducted on 24 cases of acetabu]ar fracture with central dislocation of the hip joint which were treated with anterior and posterior approaches from Jun. 2008 to Dec. 2011. All the patients were treated with open reduction and fixation with reconstruction plate and screw. Results All the patients were followed up for 6 to 36 months(average 15 months), according to Matta standard, 15 cases were anatomical reduction, nine cases were satisfactory reduction. Function of hip joint was evaluated by modified d Aubigne and Postel hip score standards: excellent in 7 cases, good in 14 cases, fair in 3 cases, the excellent rate of 87.5%. There were traumatic arthritis in 6 cases, and heterotopic ossification in 4 cases. The iatrogenic injury of the lateral femoral cutaneous nerve occurred in three cases and all were incomplete injury, fully recovered after a month or so. Conclusion The surgical treatment of acetabular fractures with central dislocation of the hip joint, using the combined approach has the advantage of easier exposure, reduction and fixation, with fewer complications, and can obtain the satisfactory clinical efi$cacy.
