不同手术方法治疗老年股骨粗隆间骨折的疗效比较

摘　　要：目的比较动力髋螺钉(DHS)、人工股骨头置换术及外固定架治疗老年股骨粗隆间骨折的疗效。方法对138例老年股骨粗隆间骨折采用DHS内固定67例,人工股骨头置换术治疗50例,外固定架固定21例。结果术后均获随访12～22个月,平均16个月。各组疗效满意,优良率比较差异无统计学意义(P>0.05)。结论不同手术方法均可有效治疗老年股骨粗隆间骨折,但应根据患者具体情况进行综合评估,选择最合理手术方式。Objective To investigate the efficacy of dynamic hip screw, artificial femoral head replacement and external fixator for the femoral intertrochanteric fracture in elderly patients. Methods Totally 138 femoral intertrechanteric hip fractures of elderly patients with complete clinical data were retrospectively analyzed, who were treated with dynamic hip screw （67 cases）, artificial femoral head replacement（50 cases） and external fixator（24 cases）. Restflts All the patients were followed up for 16 months （ range, 12 to 22 months） and received a satisfactory effect and there were no statistical significance in the acceptable rate（P 〉0.05 ）. Conclusion All kinds of treatment arc effective for the elderly patients＂ the femoral intertrochanteric fracture. We should take the patients＂ individual conditions into account and choose a reasonable operation method.
