摘 要:目的探讨计算机辅助设计数字化个体定制钢板内固定治疗复杂胫骨平台骨折的方法及短期I临床疗效。方法对27例复杂胫骨平台骨折采用数字化个体定制钢板内固定治疗。结果术后27例均获随访12-24个月,平均14.8个月。患者自我评价按照ManNab标准:优7例,良12例,可7例,差1例,总体满意率96.30%。术后12个月膝关节功能评分采用Lysholm评分,平均(85.15+9.55)分;膝关节活动度,平均屈(114.44±31.84)。,伸(1.67±6.66)。。末次随访时采用影像学Rasumussen评分,平均(11.63±2.48)分。结论数字化定制个体钢板内固定治疗复杂胫骨平台骨折可使钢板更适合个体,骨折尽可能达到解割复位。并有效恢复平台的完整性,患者可早期行功能锻炼,为围关节骨折内固定提供一有效的方法。Objective To introduce treatment on complex tibial plateau fractures with digital and individual plates based on computer-aided design and evaluate its clinical outcome. Methods Retrospective analysis was made on 27 patients with complex tibia/plateau fractures treated with digital and individual plates. Results All of 27 patients were followed up for 12- 24 months,average 14.8 months. According to the MaeNab scores: excellent in 7 cases, good in 12 eases, fair in 7 eases, bad in 1 case, and the overall satisfactory rate was 96.30%. At 12 months after surgery, functional results showed a mean Lysholm score of (85.15±9.55)points. And the mean ROM of knee, flexion (114.44±31.84), extendation (1.67±6.66). At last follow- up, the radiologic result of mean Rasussen score was (11.63±2.48)points. Conclusion Treatment on complex tibial plateau fractures with digital and individual internal fixation based on computer-aided design can not only make internal fixation more suitable on different individuals, but also restore anatomical alignment and recovery platform integrity, meanwhile early postoperative functional exercises of knee section is allowed, clinical results are satisfactory.
