摘  要:目的:对北京地区成人呼吸道感染患者中人冠状病毒(HCoV)HKUl的感染状况进行初步分析,了解其流行分布、临床特点,并确定流行株的基因型别。方法:2011年1月~2012年1月从北京地区成人呼吸道感染患者中收集鼻咽拭子标本559份,利用靶向棘突蛋白S与核心蛋白N的2种巢式PCR方法进行HCoV—HKUl筛查,并对阳性片段进行序列测定,以确定其基因型别;同时,对阳性标本进行常见呼吸道病毒的共感染筛查,进而分析HCoV—HKUl感染的临床表现和流行病学特点。结果:559份鼻咽拭子标本中检出HCoV—HKUl阳性9例(1.61%),其中3例合并HCoV-0C43感染;感染患者临床表现为急性呼吸道症状,以发热(88.9%)、咽痛(66.7%)、寒颤(68%)、流涕(55.6%)和咳嗽(44_4%)为主,其中1例有系统性红斑狼疮史;阳性基因片段系统进化分析发现这9例感染均为HCoV—I-IKUlB型。结论:北京地区近年成人呼吸道感染患者中HCoV—HKUl感染率较低(1.61%)。其流行株基因型为B型。Objective: To study the molecular epidemiological and clinical profiles of human coronavirus(HCoV) HKU1 among adults with acute respiratory infection(ARI) in Beijing. Methods: The nest RT-PCR assay targeting to spike(S) and nucleocapsid(N)coding region of HCoV-HKU1 was developed and used for screening HCoV- HKU1 infection among 559 nasopharyngeal swab specimens, which collected from adults with ARI in Beijing be- tween January 2011 and January 2012. The demographic and clinical data associated with HCoV-HKU1 infection were examined. Co-infection with other HCoV or respiratory viruses was also investigated by RT-PCR or multiple RT-PCR. The genotype distribution of HCoV-HKU1 infection was identified by sequence analysis of PCR positive products. Results: 9 of 559(1.61%) specimens were detected as positive for HCoV-HKU1 by nest RT-PCR, 3 of them were found co-infected with HCoV-OC43. The clinical profiles were indicated as fever(88.9%), pharyngitis (66.7%), rigors(68%), nasal discharge(55.6%) and cough(44.4%), and one of 9 patients with SLE. Sequence analysis showed that all of the HCoV-HKU1 infections fell into genotype B. Conclusion: Our data showed that HCoV-HKU1 prevalence was low(1.61%) among adults with ARI in Beijing area, the genotype distribution of HCoV-HKU1 was identified as B.
