摘　　要：目的探讨两环两沟法经尿道等离子体切除术(PKRP)治疗高龄重度前列腺增生(BPH)的安全性和有效性。方法将前列腺重量大于80g的BPH患者43例分成两组,实验组21例,对照组22例。实验组先用柱状电极分别汽化上下标志沟,再在膀胱颈口与近前列腺尖部汽化两个环状创面,然后用环状电极切除左右侧叶,并修整尖部。对照组按常规方法手术。两组分别记录手术时间、术中出血量和输血量。结果随访3个月,两组患者均排尿通畅。无水中毒、尿失禁及继发性出血。两组手术时间、术中出血量及输血量,差异有显著性(P<0.05)。结论两环两沟法PKRP治疗高龄重度BPH安全有效,且可缩短手术时间和减少术中出血及输血量。[Objective] Explore the safety and efficacy of using two ring two trench operation method to treat senile and severe BPH trough PKRP. [Method] The patients （43 cases） with benign prostate hyperplasia whose prostate weighs over 80g are divided into two groups, in which there are 21cases in experiment group and 22 cases in control group. First, in experiment group we vaporize fluctuation logo trenches with ring electrode and vaporize two cricoid wound in bladder neck mouth and near prostate tip, then resect left and right side leaves with cricoid electrode and trim lips. In two groups we record the operation time intraoperative amount of bleeding and blood transfusion. [Results] Follow-up of 3 months, patients in two groups urinated fluently. There were no water intoxication, incontinence and secondary hemorrhage. The difference of operation time, amount of bleeding and transfusion in the two groups is statistically significant （P 0.05）. [Conclusion] Using the two ring two trench operation method of PKRP to treat for aged patients with severe BPH is safe and effective. It can shorten operation time and reduce intraoperative amount of bleeding and blood transfusion.
