农村中老年膝骨性关节炎患者自我管理行为的现状及相关因素分析

摘　　要：目的:探讨农村中老年膝骨性关节炎患者自我管理行为的现状及影响因素。方法:采用一般情况调查表、自我管理行为量表、关节炎自我效能量表、膝骨性关节炎指数评估表(WOMAC),对216例膝骨性关节炎患者进行调查。结果:患者自我管理各维度得分的中位数分别是体能锻炼0min/周,耐力锻炼165min/周,认知性症状管理1.17,与医生沟通0.33,自我管理行为相对较好的是认知性症状管理方面。影响自我管理得分的主要因素是自我效能、WOMAC指数、疼痛程度、病程、对骨关节炎是否重视及BMI指数。结论:农村中老年膝骨性关节炎患者自我管理水平均处于较低水平,应针对影响因素施以干预,从而学会正确有效的自我管理方法,缓解病情,提高生活质量。Objective To investigate the status and influencing factors of self-management among middle-aged and elderly pa- tients with knee osteoarthritis in the rural area. Methods Totally 216 knee osteoarthritis patients were investigated with the demo- graphic data questionnaire, the Self-Management Behavior Scale, the 8-item Arthritis Self-Efficacy Questionnaire and the Western Ontario and McMaster Universities （WOMAC） Osteoarthritis Index. Results The median score of self-management in strengthe- ning exercises was O,in aerobic exercise was 165,in cognitive symptoms management practice was 1.17,in communication with physicians was 0.33. The average level of cognitive symptoms management practice was relatively high in the four aspects. Self-ef- ficacy, WOMAC index,levels of their Pain, course of the disease, attention to the disease or not and BMI index were the main fac- tors of self-management behavior. Conclusion The level of self-management among middle-aged and elderly patients with KOA in the rural area was lower and interventions targeted at the influencing factors of self-management should be enhanced.
