摘  要:目的 探讨腹腔镜子宫肌瘤剔除术中垂体后叶素或缩宫素的临床止血效果。方法 将昆明医科大学第二附属医院妇科2008年6月-2012年12月行腹腔镜子宫肌瘤剔除术的79例患者,随机分为垂体后叶素组(采用垂体后叶素加0.9%氯化钠注射液子宫肌层注射)和缩宫素组(采用缩宫素子宫肌层注射)。结果 术中用药15min后,分别观察2组患者出血量、手术时间及血压垂体后叶素组术中出血量65.2±22.3 mL,手术时间为55.3 ±25.2min;缩宫素组术中出血量98.3±32.3mL,垂体后叶素组手术时间为55.3 ±25.2min,而缩宫素组手术时间为80.2±26.3min。垂体后叶素组患者血压(收缩压)较缩宫素组明显升高10-30mmHg, 30min后逐渐降至正常。结论 腹腔镜子宫肌瘤剔除术中应用垂体后叶素,可明显减少术中出血,缩短手术时间,其对患者血压的影响是短暂的,值得临床推广和应用。Objective To explore the clinical hemostatic effect of laparoscopic myomectomy in pituitrin or oxytocin. Methods The 79 cases with intramural hysteromyoma from June 2008 to Dec. 2012 in the?Second Affiliated Hospital of Kunming Medical University were analyzed in this study(40 cases in pituitrin group, 39 cases in oxytocin group). The cases in pituitrin group were injected with pituitrin + 0.9%NS and the ones in oxytocin group were given with oxytocin. Result 15 minutes after the injection during the operation, The?two?groups?of?patients?were?observed?for?the?treatment?effect,?The blood loss of the pituitrin group(65.2±22.3mL) were significantly less than that of oxytocin group(98.3±32.3mL).The blood pressure increased 15min after pituitrin injection in the pituitrin group, which was obviously higher 10~30mmHg than that of oxytocin group, 30 min after gradually drop to normal. Conclusion Laparoscopic myomectomy application pituitrin can significantly reduce the intraoperative bleeding and shorten the operation time. Furthermore, the influence on blood pressure is temperary. It is worth promoting and utilizing in clinic environment.
