干扰素-α致老年丙型肝炎患者抑郁与Th1/Th2型细胞因子的相关分析

摘　　要：目的探讨干扰素-α致老年丙型肝炎患者抑郁症时Th1/Th2细胞因子失衡的变化情况。方法用半定量逆转录聚合酶链反应技术检测20例正常对照、54例丙型肝炎患者干扰素-α治疗12周时外周血单个核细胞内干扰素(IFN-γ)、肿瘤坏死因子-α(TNF-α)、白介素-4(IL-4)、白介素-10(IL-10)mRNA表达水平变化情况,同时应用Zung抑郁症自测量表(ZDS)对患者进行心理评估,然后对两组数据分析比对,进行相关性研究。结果老年女性丙型肝炎患者经过干扰素-α治疗后,抑郁积分数值明显增加,与中青年女性组患者以及老年男性组患者相比有显著性差异,且存在Th1/Th2细胞因子失衡,倾向于Th1,两者之间存在一定相关性。结论干扰素-α治疗丙型肝炎过程中,在老年女性患者更容易出现Th1/Th2细胞因子失衡和抑郁症状。Objective To explore the relation between the depressive symptoms and Tbl /Th2 cytokines thyroxin in elder female patient suffering from chronic hepatitis C administrated with PEG - IFN -α2a. Methods 54 patients suffering from chronic hepatitis C were divided into 3 groups, according to the age and gender,Group of older female, Group of older male, Group of Middle - aged female. Thl/Th2 cytokines were detected in blood serum by RT - PCR, and a psychological evaluation was done by Zung Self - Depression Scale （ZDS） before treatment and After 12W treatmented with IFN - ct. Results TH1 cytokines were significantly decreased in group of older female compared with other two groups （ P 〈 0.05 ）, but there were not significantly difference onTH2 cytokines among the three groups. Different degrees of depression symptoms in these patients were found after treatment with PEG - IFN-α2a. Scores of older female patients were more significantly increased than the older men group and middle- aged female patients （ P 〈0. 001 ）. Zung quantified scores were also significantly increased in the older female group before and after treatment （ P 〈 0.05 ）. In addition, between the extent to depression and the level TH1 cytokines in the older female patients, the exist- ence of disease - related trends was found. Conclusion The elder female patients suffering from chronic hepatitis C administrated with PEG - IFN -α2a have more prone to depression, and TH1/TH2 cytokines imbalance.
