摘 要:目的分析脑外伤合并骨折时低氧诱导因子-1对骨折愈合的影响效果。方法选用72只健康大鼠,随机分为骨折组(F组)和脑外伤合并骨折组(BF组),先后制作脑外伤模型与骨折模型,进行苏木精-伊红(HE)染色和组织免疫化学染色,对两组大鼠低氧诱导因子-1α(HIF-1α)表达情况进行分析。结果 BF组的大鼠在伤后第1周HIF-1α阳性细胞明显增多,骨细胞增长明显,成纤维细胞比较活跃;平均灰度值、平均光密度值和积分光密度值均呈现出最大值,提示骨痂组织矿化比较明显;在伤后的第2~4周,骨细胞和软骨细胞整体数量增加,成纤维细胞相对减少,但仍较为活跃。F组大鼠在伤后的第1周提示存在成纤维细胞,但直至第4周骨细胞和软骨细胞总体数量仍较少;伤后1~4周HIF-1α阳性细胞以及平均灰度值、平均光密度值和积分光密度值变化均不明显。结论低氧诱导因子-1在促进骨痂组织恢复中作用明显,在防止骨质疏松及治疗骨折延迟愈合等方面可通过激活低氧诱导因子-1改善临床整体效果。Objective To analyze the effect of hypoxia inducible factor - 1 ( HIF - 1 ) on fracture healing when brain injury combined with fracture. Methods Seventy - two healthy rats were divided into fracture group ( group F) and brain trauma combined with fracture group (BF group) randomly. According to Feeney free -fall model and GaoYan's modification, rats brain injury combined with fracture model were made. HE staining and immunochemistry staining for HIF -1α were performed respectively. The HE staining results and expression of HIF - 1 in two groups of rats were compared. Results For BF group, in the first week after injury, HIF - 1 ct positive cells was significantly increased, obvi- ous bone cell proliferation and active fibroblasts were observed; the average gray value, average optical density value and integral optical density value all reached the maximum value, indicating obvious mineralization of callus tissue. 2 - 4 weeks after the injury, the whole number of bone cells and cartilage cells were increased, fibroblasts were relatively less, but still active in BF group. For F group, fibreblasts were found in the first week after injury, however, the bone cells and cartilage cells in the overall population were still less until 4 weeks after injury; HIF -1α positive cells and average gray value, average optical density value, and integral optical density value did not change obviously 1 -4 weeks after injury. Conclusion The study suggests that HIF - 1 promotes fracture healing during the recovery of bone fracture. The activation of HIF - 1 may be used in preventing osteoporosis and the treatment of delayed union of bone fracture.
