摘 要:目的探讨空腹血糖(FPG)及糖化血红蛋白(HbAlc)在冠心病血糖升高患者的鉴别断价值。方法收集血糖升高的冠心病患者310例,其中糖尿病组(既往有糖尿病史)196例和无糖尿病高血糖组(既往无糖尿病史)114例,正常对照组80例,入院后次日清晨分别采集空腹静脉血3 ml,检测FPG及HbA1c水平。结果糖尿病组和无糖尿病高血糖组FPG水平均高于正常对照组,差异有显著性(P<0.01),但糖尿病与无糖尿病高血糖组间FPG水平差异无统计学意义(P>0.05)。糖尿病组FPG与HbAlc水平呈正相关(r=0.85,P<0.01),糖尿病组HbAlc水平明显高于正常对照组和无糖尿病高血糖组(P<0.01),无糖尿病高血糖组与正常对照组HbAlc水平差异无统计学意义(P>0.05)。结论冠心病患者检测空腹血糖、糖化血红蛋白能明确鉴别冠心病并糖尿病和冠心病应激性血糖增高,对临床治疗提供依据。Objective To investigate the value of monitoring fasting plasma glucose (FPG) and glycosylated hemoglobin (HbA1 c) for high blood sugar differentiation in patients with coronary heart disease. Methods Three hundred and ten patients with coronary heart disease and high blood sugar were enrolled. These patients were divided into 2 groups : patients with diabetes history ( group A, n = 196) and patients without diabetes history (group B, n = 114). A control group of 80 normal cases was set. FPG and HbAlc levels were monitored. Results FPG levels in group A and group B were significantly higher than control group ( P 〈 0. O1 ), however, there was no significant difference in FPG level be- tween group A and group B ( P 〉0.05). There was a positive correlation between FPG and HbAlc in group A (r =0.85, P 〈0.01). HbAlic level in group A was significantly higher than that in group B and control group ( P 〈0. O1 ). There was no significant difference in HbAlc level between group B and control group( P 〉 0.05 ). Conclusion FPG and HbAlc monitoring can distinguish diabetes mellitus from stress - induced high blood sugar in patients with coronary heart disease and provide evidence for clinical treatment.
