摘 要:目的观察超声透皮导入牛骨形态发生蛋白对兔桡骨缺损愈合的促进作用。方法制备牛骨形态发生蛋白超声耦合凝胶剂。成年新西兰大白兔40只,制备双侧桡骨骨折模型,植入硫酸钙人工骨。以大白兔右侧肢体为实验组,使用含牛骨形态发生蛋白超声耦合凝胶剂;左侧肢体为对照组,使用不含牛骨形态发生蛋白超声耦合凝胶剂。术后7d开始超声透入,每日1次,每次20min,连续14次。治疗结束后第2、4、8、12周各处死动物10只,取材。摄X线片观察骨折愈合,并组织学检查骨痂生长情况,取第4、6周骨痂进行碱性磷酸酶和钙含量测定。结果X线检查、光镜下组织学观察表明实验组骨折愈合明显优于对照组。第12周时实验组X线检查示骨缺损区皮质骨密度接近正常皮质骨,骨髓腔完全再通,骨塑型明显,基本恢复桡骨正常形态;组织学检查示骨缺损区骨痂转化为成熟的板层骨相连,并与原有皮质骨良好融合,骨髓腔形成。治疗后第4、12周时,实验组骨痂碱性磷酸酶和钙含量明显高于对照组,差异均有统计学意义[4周:(25.60±0.51)U/mg比(5.52±0.21)U/mg,(310±21)μg/mg比(227±16)μg/mg;12周:(40.78±0.35)U/mg比(8.56±0.14)U/mg,(459±16)μg/mg比(250±16)μg/mg;均P〈0.01]。结论超声透皮导入牛骨形态发生蛋白可促进兔桡骨缺损愈合。Objective To evaluate the repair of radial defects in rabbits treated by bovine bone morphogenetic protein(bBMP) throug electrophonophoresis. Methods Ultrasonic coupling gel combined with bBMP was prepared. Forty mature New Zealand rabbits were prepared and the 10 mm-long segmental bone defects were made in both radii of each rabbit as bone defect model. Calcium sulfate artificial bone were implanted in the areas of bone defects of two groups. Right limbs were selected into experimental groups with ultrasonic coupling gel combined with bBMP and left limbs belonged to control groups with ultrasonic coupling gelcombined without bBMP. Roentgenography, histomorphometrical analysis of osseous tissue were performed to assess the degree of fracture healing in both groups at 4 th, 6 th, 8 th and 12 th week after treatments. Alkaline phosphatase(ALP) activity and calcium content in callus were measured. Results All results showed that callus formation and bone linking in experimental group were improved significantly than the control group. ALP activity and calcium content in experimental group were significanty higher than that in control group[4th week: (25.60 ±0.51 )U/mg tissue vs (5.52 ± 0.21 )U/mg tissue, (310 ±21)μg/mg tissue vs (227 ± 16)μg/mg tissue; 12th week: (40.78 ± 0. 35)U/mg tissue vs (8.56 ± 0.14) U/mg tissue, (459 ± 16) μg/mg tissue vs (250 ± 16) Ixg/mg tissue; all P 〈 0.01 ]. Conclusion The electrophonophoresis combined with bBMP has a positive effect on the whole process of fracture healing.
