局部0．25％布比卡因用药时间对腹腔镜胆囊切除术后切口疼痛的影响

摘　　要：目的探讨腹腔镜胆囊切除术（LC）中4个切口局部浸润0．25％布比卡因时间对术后切口疼痛的影响。方法采用随机数字表法将75例LC患者分为A、B和对照组，各25例。A组在麻醉后切皮前以3ml 0．25％布比卡因分别对4个切口（分别为剑突下、右腋前线肋缘下、右锁骨中线肋缘下以及脐孔）进行全层浸润；B组在手术结束后患者清醒前进行局部浸润；对照组不予局部浸润。术后均不给予镇痛药。采用视觉模拟评分（VAS）评价镇痛效果。结果术后1、2、4、6、8h，B组剑突下切口的静态VAS[分别为（1．9±1．7）、（1．6±1．4）、（1．2±1．1）、（0．8±0．7）、（0．6±0．5）分]明显低于对照组[分别为（4．6±1．9）、（3．8±2．1）、（2．6±1．7）、（2．1±1．5）、（1．5±1．3）分]（均P〈0．05）；术后1、2、4、6、8、24h，B组动态VAS[分别为（2．7±2．4）、（2．4±2．2）、（1．8±1．6）、（1．5±1．3）、（1．1±1．0）、（0．9±0．8）分]明显低于对照组[分别为（5．7±2．4）、（5．0±2．6）、（3．5±1．9）、（2．9±1．9）、（2．4±2．3）、（2．3±2．2）分]（均P〈0．05）。结论LC患者清醒前给予0．25％布比卡因局部浸润可以明显减轻剑突下切口术后疼痛。Objective To investigate the effect of 0.25 % bupivaeaine infiltration of troear sites at different times on postoperative pain after laparoseopie eholeeysteetomy（LC）. Methods Seventy-five patients scheduled for selective LC were randomized into three groups with 25 eases each. The four troear sites were infiltrated with 3 ml of 0.25% bupivacaine just before the operation（group A）or just at the end of the operation （group B）. Group C was not given bupivaeaine infiltration as the control. Postoperative pain visual analogue scale （VAS）was recorded at rest and during movement. No patients were administered analgetiea. Results For the subxiphoid incision, VAS at rest in group B [ 1 h（ 1.9 ± 1.7） scores, 2 h（ 1.6 ± 1.4） scores, 4 h （ 1.2 ± 1.1 ） scores, 6 h （0.8 ± 0.7） scores, 8 h （0.6 ± 0.5 ） scores ] were siguifieantly lower than those in group C [ 1 h （4.6 ± 1.9 ） scores, 2 h （ 3.8 ± 2.1 ） scores, 4 h（2.6 ± 1.7） scores, 6 h（2.1 ± 1.5 ） scores, 8 h （ 1.5 ± 1.3 ） scores ] after the operation （ P 〈 0.05 ）. While VAS during movement in group B [ 1 h（2. 7 ± 2.4） scores, 2 h （2.4 ± 2.2 ） scores, 4 h （ 1.8 ± 1.6） scores, 6 h （ 1.5 ± 1.2 ） scores, 8 h （ 1.1 ± 1.0 ） scores, 24 h （ 0.9 ± 0.8 ） scores ] were siguifieantly lower than those in group C [ 1 h （5.7 ± 2.4） scores, 2 h （5.0 ± 2.6） scores, 4 h （3.5 ± 1.9） scores, 6 h （2. 9 ± 1.1 ） scores, 8 h（2.4 ± 2.3 ） scores, 24 h（2.3 ±2.2） scores] after the operation（P 〈0.05）. Conclusion The infiltration of 0. 25% bupivaeaine just at the end of operation at the subxiphoid incision can effectively decrease the postoperative ineisional pain.
