摘  要:目的调查耐药鲍氏不动杆菌水平获得性耐药相关基因与可移动遗传元件的关系。方法收集2009年1-12月医院住院患者痰标本中分离的耐药鲍氏不动杆菌共20株,采用聚合酶链反应(PCR)方法分析54种水平转移获得的β-内酰胺类、氨基糖苷类、喹诺酮类获得性耐药基因和12种接合性质粒、转座子、插入序列、整合子等可移动遗传元件遗传标记检测以及插入序列与β-内酰胺酶基因(ISaba1与ADC、ISaba1与OXA-23)连锁检测,再对检测结果作指标聚类分析(UPGMA法)。结果 20株耐药鲍氏不动杆菌共检出4种获得性β-内酰胺类耐药基因(TEM、PER、ADC、OXA-23),5种获得性氨基糖苷类耐药基因(aac(3)-Ⅰ、aac(6′)-Ⅰb、ant(3″)-Ⅰ、aph(3′)-Ⅰ、armA),2种抗菌制剂外排泵基因(adeB、qacE△1),7种可移动遗传元件的遗传标记(intⅠ1、tnpU、tnp513、IS26、IS903、ISaba1、ISaba9);指标聚类分析提示,获得性耐药基因armA、ADC、ant(3″)-Ⅰ、qacE△1、adeB、TEM、aac(3)-Ⅰ与可移动遗传元件tnpU、IS26、intⅠ1、tnp513、ISaba1高度相关,阳性率均>90.0%;插入序列与β-内酰胺酶基因(ISaba1与ADC、ISaba1与OXA-23)连锁检测均呈阳性。结论该组耐药鲍氏不动杆菌获得性耐药基因与可移动遗传元件有一定的关联度,菌株携带的获得性耐药基因可能由可移动遗传元件介导。OBJECTIVE To investigate the correlation between the horizontal acquired resistance genes and mobile genetic elements in drug-resistant Acinetobacter baumannii(A.baumannii).METHODS From Jan 2009 to Dec 2009,20 strains of drug-resistant A.baumannii were collected from a hospital in China.Then,the polymerase chain reaction(PCR) was employed to analyze 54 kinds of acquired resistance genes to beta-lactams,aminoglycosides,quinolones and 12 kinds of genetic markers of conjugal plasmids,transposons,insertion sequences,and integrons as well as linked detection of insertion sequences and beta-lactamases(ISaba1 and ADC,ISaba1 and OXA-23),then the index cluster analysis(UPGMA) of the detection result was performed.RESULTS Of 20 strains of pandrug-resistant A.baumannii,4 kinds of acquired beta-lactam-resistance genes(TEM,PER,ADC,OXA-23),5 kinds of acquired aminoglycoside-resistance genes(aac(3)-Ⅰ,aac(6′)-Ⅰb,ant(3″)-Ⅰ,aph(3′)-Ⅰ,armA),2 kinds of drug efflux genes(adeB,qacE△1),7 kinds of genetic markers of mobile genetic elements(intⅠ1,tnpU,tnp513,IS26,IS903,ISaba1,ISaba9) have been detected,and the index cluster analysis showed there was high-level correlation between the acquired resistance genes(armA,ADC,ant(3″)-Ⅰ,qacE△1[J],adeB,TEM,aac(3)-Ⅰ) and genetic markers of mobile genetic elements(tnpU,IS26,intⅠ1,tnp513,ISaba1) with the positive rates higher than 90.0%.Furthermore,linked detection of insertion sequences and beta-lactamases(ISaba1 and ADC,ISaba1 and OXA-23) were all detected positive.CONCLUSION In this group of drug-resistant A.baumannii,there is certain correlation between the acquired resistance genes and mobile genetic elements,the acquired resistance genes may be mediated by mobile genetic elements.
