丙种球蛋白治疗老年患者脊柱固定术术后细菌感染的疗效分析

摘　　要：目的观察丙种球蛋白治疗脊柱固定术老年患者术后细菌感染的临床疗效,并分析与探讨其临床价值。方法将医院自2008年3月-2012年8月收治的脊柱固定术后细菌感染的103例老年患者随机分为两组,对照组52例行常规综合疗法,观察组51例在对照组治疗方法的基础上应用丙种球蛋白治疗,对比两组患者住院治疗、抗菌药物静脉用药时间及出院时总有效率及痊愈率;随访3～5年,对比两组患者再次感染率。结果对照组住院治疗、抗菌药物静脉用药时间分别为(21.2±8.7)、(19.7±8.3)d,观察组为(17.3±7.9)、(15.8±7.5)d,观察组患者住院治疗和抗菌药物静脉用药时间较对照组明显缩短(P<0.05);两组患者出院时总有效率均为100.0%;但对照组和观察组痊愈率分别为80.8%和92.2%,两组间痊愈率差异有统计学意义(χ2=5.176,P<0.05);对照组和观察组均未发生再次感染。结论应用丙种球蛋白治疗脊柱固定术老年患者术后细菌感染,能缩短其治疗时间,从而减少抗菌药物用药量,且疗效确切、痊愈率高,值得临床推广。OBJECTIVE To observe the clinical efficacy of gamma globulin auxiliary in the treatment of elderly patients with bacterial infections after the spine fixation so as to analyze and discuss its clinical value.METHODS In the hospital,totally 103 elderly patients suffering from this disease were divided into the observation group and the control group,52 patients of the control group underwent the conventional combination therapy based on the specific circumstances,51 patients of the observation group used gamma globulin auxiliary simultaneously.The hospitalized treatment,time of intravenous use of antibiotics,the total effective rate on the discharge,and the cure rate of the two groups were compared and observed.RESULTS The hospitalized treatment time was（21.2±8.7）d in the control group,（17.3±7.9）d in the observation group;the time of intravenous use of antibiotics was（19.7±8.3）d in the control group,（15.8±7.5）d in the observation group,both significantly shorter in the observation group than in the control group（P0.05）;the total effective rates of the two groups on the discharge were 100.0%,but the cure was 80.8% in the control group,92.2% in the observation group,the difference in the cure rate between the two groups was statistically significant（χ2=5.176,P=0.03）;the infections occurred again in neither the control group nor the observation group.RESULTS CONCLUSION The gamma globulin,as in the treatment of elderly patients with bacterial infections after the spine fixation,can shorten the treatment time and reduce the dosage of antibiotics,with the clinical efficacy accurate,the cure rate high,which is worthy of being promoted in the hospital.
