中医综合保守疗法治疗退行性腰椎滑脱症61例临床观察

摘　　要：目的观察中医综合保守疗法治疗退行性腰椎滑脱症的临床疗效。方法将121例退行性腰椎滑脱症患者随机分为中医组61例和西医组60例,中医组采用中药颗粒剂内服治疗、手法治疗和练功,西医组口服氨糖美辛胶囊、腰部牵引、佩戴腰围,两组共治疗2周。于治疗前后、随访时(疗程结束后1个月)进行JOA评分和VAS评分,X线测定平行、角度位移,于治疗前后各检查1次,并观察临床疗效。结果两组在VAS评分及JOA评分上,治疗后及随访时与治疗前比较均明显降低(P<0.01)。中医组治疗后和随访时总有效率分别为93.5%、95.1%,西医组治疗后和随访时总有效率均为81.7%,中医组疗效优于西医组(P<0.01)。两组患者治疗前后滑脱腰椎平行位移及角度位移比较差异无统计学意义(P>0.05)。结论中医综合保守疗法治疗退行性腰椎滑脱症疗效肯定,并且优于西医综合保守疗法。Objective To observe the therapeutic effect of TCM comprehensive conservative therapy for degenerative spondylolisthesis. Methods Totally 121 cases of degenerative spondylolisthesis were randomized into the TCM group and western medicine （WM） group, with 61 and 60 in each. The TCM group was treated with TCM granules, manual therapy and exercises. The WM group was treated with Glucosamine Indomethacin Capsules, waist traction and wearing waist trimmer belt. Both groups were treated for 2 weeks. The Japanese Orthopaedic Association （JOA） scoring system and Visual Analogue Scale （VAS） was marked before and after treatment and during the follow-up （one month after the end of treatment）. The parallel and angular displacement was checked by X ray determination once before and after treatment. The therapeutic effect was observed. Results The VAS scores and JOA scores were significantly decreased in both groups after treatment and during the follow-up （P〈0.01）. The total effective rate after treatment and during the follow-up was 93.5% and 95.1% in the TCM group, while 81.7% and 81.7% in the WM group, with a significant difference between groups （P〈0.01）. There was no significant difference between before and after treatment in the parallel and angular displacement of spondylolisthesis in both groups （P〉0.05）. Conclusion TCM comprehensive conservative therapy is effective for degenerative spondylolisthesis with a better therapeutic effect than WM conservative therapy.
