HTK液中添加聚合胎盘血红蛋白对离体鼠心心肌保护效果的价值研究

摘  要:目的探讨在冷缺血期间HTK液加入0.1%聚合胎盘血红蛋白(0.1g/100ml HTK solution,polymerized placenta hemoglobin,PPHb)对离体鼠心的心肌保护效果。方法成年雄性Wistar大鼠56只,根据保存液和保存时间的不同,随机分为7组:对照组,HTK液中分别保存3h组、6h组和8h组;HTK+0.1%PPHb(HTKHb)液中分别保存3h组,6h组和8h组,每组8只。除对照组外,其他6组均在4℃的两种不同保存液中分别保存3h,6h和8h。然后建立Langendorff模型,观察左心室功能、冠状动脉流量(CF)、冠状动脉流出液中乳酸脱氢酶(LDH)、乳酸脱氢酶同工酶1(LDH1)、肌酸激酶(CK)及肌酸激酶同工酶(CKMB)的含量及心肌组织的病理改变。结果在HTK和HTKHb保存液中,保存3h再灌注后,HTKHb-3h组LVDP,dp/dtmax,dp/dtmin及心肌ATP含量高于HTK-3h组(P<0.05),CK-MB的漏出量低于HTK-3h组(P<0.05);保存6h再灌注后,各项观察指标相比无统计学差异;而保存8h再灌注后,HTKHb-8h组LVDP,dp/dtmax,dp/dtmin及心肌ATP含低于HTK-8h组(P<0.05),CK和CK-MB漏出量高于HTK-8h组。结论 HTK液中添加0.1%PPHb可以提高离体鼠心低温保存3h的保存效果;但是保存时间延长至8h,加重了心肌组织损伤。Objective To evaluate the myocardial protective effect of HTK solution combined with 0.1% polymerized placenta hemoglobin(0, lg/100ml HTK solution) on the isolated rat heart. Methods The 56 adult male Wistar rats were randomly divided into 7 groups according to the different preservation solutions or different preservation times: control group;3-hour group,6-hour group, 8-hour group preserved in HTK solution;the other 3-hour group,6-hour group;8-hour group preserved in HTK solution combined with 0.1% PPHb (HTKHb), with 8 rats in each group. Except for control group, the isolated rat hearts in the other 6 groups were preserved in the HTK or HTKHb solution at 4℃ for 3,6,8 hours, respectively. Then the Langendorff models were established, and the changes of left ventricular function, coronary artery flow (CF) and pathological structure of myocardium were observed,and the enzyme contents released by myocardium including LDH, LDH1, CK, CKMB were detected. Results After the isolated rat hearts were persevered in the HTK or HTKHb solution for 3 hours and were re-perfused for 1 hour, the cardiac indexes including LVDP, dp/dtmax, dp/dtmin and ATP contents in myocardium were significantly higher than those in HTK 3-hour group ( P 〈 0. 05 ), however, the outleakage of CK-MB was significantly lower than that in HTK 3-hour group. After 6-hour preservation, there were no significant differences in the cardiac indexes between the two groups. However, after 8-hour preservation, the cardiac indexes including LVDP, dp/dtmax, dp/dtmin and ATP contents in myocardium in HTKHb 8-hour group were significantly lower than those in HTK8-hour group( P 〈 0.05 ), but the outteakage of CK-MB was significantly higher than that in HTK 8-hour group. Conclusion HTK solution combined with 0.1% PPHb can improve the myocardial protective effect during 3-hour hypothermic preservation. However, the myocardial injury may be aggravated during 8-hour hypothermic preservation.
