摘 要:目的:以三维实验结果为标准,比较表型筛选法和双抑制剂扩散协同法试验检测产AmpC酶阴沟肠杆菌的可靠性。方法:收集2011年2月-2012年12月我院临床分离的阴沟肠杆菌39例,分别用三维实验、表型筛选法、双抑制剂扩散协同法试验检测产AmpC酶阴沟肠杆菌。结果:39株阴沟肠杆菌经三维试验检测出高产AmpC酶的阴沟肠杆菌10株,检出率为25.6%。表型筛选法特异性为69.0%,灵敏度为90.0%,检出效率为76.9%。双纸片协同试验特异性为93.1%,灵敏度为80.0%,检出效率为89.7%。结论:双抑制剂扩散协同法具有较高的灵敏度和特异性,且与三维试验阳性检出率相当,是一种可靠的方法。Objective: To compare the reliability of phenotypic screening method and dual inhibitors diffuse synergy test in detecting AmpC enzymes produced by Enterobacter cloacae based on the results of 3D experiment. Methods: Collecting 39 cases who clinically isolated Enterobacter cloacae in our hospital from February 2011 to December 2012, detecting AmpC enzymes produced by Enterobacter cloacae with three-dimensional experiment, phenotypie screening method and dual inhibitors diffuse synergy test, respectively. Results: 10 cases highly produced AmpC enzymes in 39 Enterobacter cloacae by three-dimensional experiment, the detection rate was 25. 6%. The specificity, sensitivity and detection efficiency for phenotypic screening method were 69. 0%, 90. 0%, 76. 9%, respectively. And that for double disk synergy test were 93. 1%, 80. 0%,89. 7 %, respectively. Conclusion: Dual inhibitors diffuse synergy test is a reliable method for its high sensitivity and specificity, and it is consistent with the positive detection rate of three-dimensional test.
