HIF-1α在原发性系膜增生性肾小球肾炎的表达以及与中医辨证分型的相关性

摘　　要：目的:研究缺氧诱导因子-1(HIF-1)在系膜增生性肾小球肾炎中的表达趋势及MsPGN与中医辨证分型的相关性。方法:回顾性的分析我院收治的原发性MsPGN120例,根据中医辨证分为四型。随机选取40块肾穿组织标本,应用免疫组化Envision二步法检测标本肾小球及肾小管区HIF-1α表达变化情况,采用HPIAS-2000型高清晰度彩色病理图像分析系统半定量分析肾小球和肾小管间质特异染色部分的相对面积,同时进行统计学分析。结果:系膜增生性肾小球患者肾穿组织中HIF-1α随着肾小球系膜增生性病变加重在肾小球肾小管区表达增强,而且在肾小球硬化出现片状表达。MsPGN中医辨证分型中,肺肾气虚型与脾肾阳虚型中HIF-1α在肾小球的表达各与其他三型差异有统计学意义(P<0.05);气阴两虚型与肝肾阴虚型中HIF-1α在肾小球的表达差异无统计学意义(P>0.05)。结论:HIF-1α与MsPGN病变程度相关;不同病理类型的MsPGN患者,可以有不同的证侯,提示在治疗中应该注意把握。Objective： Study of hypoxia inducible factor - 1 （ HIF - 1 ） expression in mesangial proliferative glomerulone- phritis trends and MsPGN TCM syndrome type correlation. nethods：A retrospective analysis of the hospital ＇s primary MsPGN 120 eases , divided into four types according to TCM. Leniently the patients with in severe MsPGN , renal biopsy tissue paraffin blocks , randomly selected 40 renal biopsy tissue samples , the application of the two - step immunohistochemical Envision detected with mild to severe MsPGN renal biopsy tissue samples glomerular and renal district HIF - 1 ct expression changes , using a semi - quantitative analysis of HPIAS - 2000 high definition color pathology image analysis system part of the relative area of specific staining of glomeru- lar and tubular interstitial , and statistical analysis . Results： HIF - 1 α in mesangial proliferative glomerulonephritis in patients with renal biopsy tissue with mesangial proliferative lesions increase in glomerular renal tubular expression increase. TCM Syndrome Types in MsPGN , lung and kidney qi deficiency type of HIF - 1α in glomerular expression of the other three types are different （P 〈 0.05） ; the Spleen deficiency in HIF -let in glomerular expression of the other three types have a significant difference （P 〈0.01 ） ; Yin Deficiency type of liver and kidney in HIF - let expression in glomerular no significant difference （P 〉 0.05 ）. Conclusion： HIF - 1 α with MsPGN severity ; the pathological type MsPGN patients may be a different card Hou , prompt treatment should pay at- tention to.
