摘 要:目的:探讨MYH9基因多态性与内蒙古地区慢性肾脏病(chronic kidney disease,CKD)患者病情进展的相关性。方法:收集301例CKD患者作为病例组,经体检证实的294例健康者作为正常对照组。采用PCR的方法分析MYH9基因Rs3752462、Rs4821480两位点基因多态性,分别比较两位点基因型及等位基因在CKD患者与对照组的差异,分析不同基因型与CKD患者发病年龄、血压、24h尿蛋白定量、血肌酐、肾小球滤过率的相关性,分析Rs3752462位点不同基因型与CKD病情进展的关系。使用SPSS16.0统计软件进行数据分析。结果:Rs3752462、Rs4821480位点符合群体遗传平衡。单因素分析显示CT基因型患者的收缩压(147.94±27.40)高于CC基因型的患者(136.43±19.09)(P<0.05);CC基因型患者的肾小球滤过率(32.00±29.98)高于CT基因型的患者(21.62±23.67)(P<0.05)及TT基因型的患者(21.99±24.18)(P<0.05)。校正年龄,采用Logistic回归显示Rs3752462位点CC基因型是造成CKD病情发展至终末期肾衰竭(end stage renal disease,ESRD)的独立保护因素。结论:MYH9基因多态性与内蒙古地区慢性肾脏病具有相关性,Rs3752462位点CC基因型是CKD病情发展至ESRD的独立保护因素,等位基因C突变为T可能导致ESRD的发生。ABSTRACT Objective: To explore the association between MYH9 gene single nucleotide polymorphism and progression of chronic kidney disease in Inner Mongolia patients. Methods: Choose 301 patients with chronic kidney disease as the case group and 294 healthy people who are confirmed to be healthy by the physical examination as normal control group The SNP in MYH9 gene of Rs3752462.Rs4821480 site were determined by PCR- restriction fragment length polymorphisms (PCR -RFLP). Compare the differences of genotype and the allete frequencies of the two sites in chronic kidney disease and the control group. Analysis the associa- tion of different genotypes and clinical datas of CKD, for example, age, blood pressure, urinary protein in 24 hour, serum creatinine and glomerular filtration rate. Analysis the relationship between different genotypes and the progression of chronic kidney disease in Rs3752462 site. SPSS16.0 statistical software is used for data analysis. Results: Rs3752462 and Rs4821480 site groups conform to the law of population genetic equilibrium( P 〉0.05 )Single factor analysis showed that systolic blood pressure of CT genotype patients is higher( 147.94± 27.40)than that of CC genotype patients( 136.43 + 19.09)( P 〈 0.05 ). Glomerular filtration rate of CC (32.00 ±29.98 ) genotype patients is higher than that of CT ( 21.62 ± 23.67 ) ( P 〈 0.05 ) and TT ( 21.99 ±24.18 ) genotype patients ( P 〈 0.05 ). After adjustment for age, logistic regression show that CC genotype is the independent protective factors for the progression from CKD to ESRD. Conclusion :There are the association between MYH9 gene polymorphism and progression of chronic kidney disease in Inter Mongolia patiants, CC genotype is the independent protective factors for the progression from CKD to ESRD, Allele C mutation for T can lead to ESRD.
