摘 要:目的:探讨声辐射力脉冲成像(acoustic radiation force impulse,ARFI)技术定量分析在糖尿病肾病(diabetic nephropathy,DN)中的临床应用价值。方法:选取35例经肾脏穿刺活检的DN患者作为研究对象(结合DN入选标准及其常规声像图特点将其分为DN-1组及DN-2组两组),对其肾脏实质及肾窦区组织弹性变化进行ARFI定量分析,并和30例正常对照组比较,比较分析各组与对照组间差异。结果:两组间肾脏的剪切波速度(shear wave velocity,Vs)均表现为肾皮质区>肾髓质区>肾窦区;DN-2组患者双肾肾皮质区、肾髓质及肾窦区Vs均较对照组减低(P<0.05);DN-1组患者双肾肾皮质区Vs较对照组减低(P<0.05),但肾髓质及肾窦区Vs与对照组间差异无统计学意义(P>0.05)。结论:ARFI技术有助于量化评估DN组织病变的损害程度,为该类疾患诊断提供一种新的定量评估指标。Objective:To assess the clinical value of acoustic radiation force impulse (ARFI) in quantitative evaluating renal tissue elastic compliance of diabetic nephropathy (DN). Methods:Quantitative evaluation of the elasticity change of renal parenchyma and sinus district organization, including 30 cases in the control group and 35 cases in DN group confirmed by renal biopsy pathology , which were divided into DN -1 and DN -2 according to the inclusion criteria and conventional uhrasonographic characteristics. Results:In all cases, the Vs was the highest in renal cortex. In DN -2 group, the Vs was lower in kidney renal cortex, medulla and sinus area than the control group ( P 〈 0.05 ). In DN - 1 group the Vs in rena cortex was lower than the control group ( P 〈 0.05), but there had no significant differences between the two groups in the renal medulla and sinus (P 〉 0.05 ). Conclusion:ARFI can quantitatively study the pathological changes of tissue elastic compliance of DN, providing a new quantitative evaluation index for diagnosing these disease.
