慢性肾脏病患者手术麻醉深度监测指标的评价

摘　　要：目的探讨麻醉深度监测对提高麻醉质量的影响，加强慢性肾脏病患者麻醉管理。方法将慢性肾脏病患者随机分为A、B两组。A组凭借麻醉医师经验判断麻醉深度及根据脑电双频指数调整麻醉药用量。B组根据麻醉深度指数、听觉诱发电位指数、皮肤电传导调整麻醉药用量。然后测量各自相关指标，并分析其与麻醉深度的相关性。结果听觉诱发电位指数能反映脑干、听觉、神经损伤的麻醉深度情况及意识的存在与消失，麻醉深度指数能反映大脑皮层意识消失后麻醉深度情况，皮肤电传导能反映意识消失前麻醉深度情况。结论联合听觉诱发电位指数、皮肤电传导和麻醉深度指数则可提高判断麻醉深度的准确性。Objective Investigating the affect by monitoring the depth of anesthesia to improve the quality of anesthesia, and to strengthen the management of anesthetic＇ patients with chronic kidney disease. Methods Chronic kidney disease patients were randomly divided into A, B two groups, A group to determine the depth of anesthesia by the experience of the anesthesiologist and bispectral index（BIS） and to adjust the anesthetic dosage, B group according to the significance of cerebral state index（CSI）, auditory evoked potentifl index （AEP index）, skin conductance（SC） to adjust the anesthetic dosage. And then to measure the respective indicators and analyze the correlation between the depth of anesthesia. Results The AEP index can reflect the depth of anesthesia in brainstem,auditory,nerve in jury,and the existence and disappearance of awareness. CSI can reflect the depth of anesthesia in the cerebral cortex,after loss of consciousness. SC can reflect the depth of anesthesia prior to loss of consciousness. Conclusions Jointing AEP index, SC and CSI, you can improve the accuracy of sentencing depth of anesthesia.
