阶梯式亚低温治疗单纯弥漫性轴突损伤患者的护理

摘　　要：目的探讨阶梯式实施亚低温治疗单纯性弥漫性轴突损伤（DAI）的护理措施。方法回顾性总结18例采用阶梯式亚低温治疗单纯眭DAI患者的护理，主要包括：阶梯式亚低温实施之前的伤情评估、阶梯式亚低温治疗期间的护理、复温后昏迷患者的护理、昏迷结束后患者的护理等。结果18例患者的颅内压、血脑脊液乳酸浓度减轻，额角扩大，功能障碍评分（DRS）明显降低，不增加亚低温并发症发生率，取得了理想的治疗效果。结论针对不同病情、不同阶段，严密而精心的护理是实施阶梯式亚低温治疗单纯性DAI的首要保证。Objective To explore the nursing measures of patients with pure diffuse axonal injury （DAI）treated by muhi-step sub-hypothermia therapy. Methods The nursing care of pure DAI patients treated by multi- step sub-hypothermia therapy, which included the traumatic condition evaluation before treatment, the nursing care during treatment, the nursing care after treatment but also in coma, and the nursing care after palinesthesia. Results The intracranial pressure and concentration and lactic acid in blood and cerebrospinal fluid alleviated. The forehead expanded. The disability rating scale（DRS）decreased. While the in- cidence of sub-hypothermia related complications did not increased. Conclusions The elaborative nursing care aiming at different pathogenetic conditions, different stages is the first guarantee in the treatment of pure DAI with multi-step sub-hypothermia therapy.
