徐长卿丹皮酚关节内注射对关节软骨影响的实验研究

摘　　要：目的通过对徐长卿丹皮酚关节腔注射后兔膝骨性关节炎模型的病理学和超微结构观察,探讨徐长卿丹皮酚是否具有修复骨性关节炎中退变关节软骨的作用。方法 40只雄性大耳白兔随机分为正常组、对照组、生理盐水组、曲安奈德组和徐长卿丹皮酚组。随机选取8只作为正常组,其余32只采用家兔膝关节前交叉韧带切断(ACLT)及内侧半月板前1/3切除制作骨关节炎(OA)模型方法进行造模,4周后造模成功。生理盐水组、曲安奈德组、徐长卿丹皮酚组分别膝关节内注射0.9%生理盐水、曲安奈德、徐长卿丹皮酚各0.2mL,每周2次,共10次。治疗完成1周后处死,暴露膝关节腔,大体观察关节软骨病理学变化;取材后在电镜下观察超微结构的改变。结果徐长卿丹皮酚组的大体标本观察软骨损伤分级优于曲安奈德组(P<0.05);电镜超微结构观察的病理改变明显轻于曲安奈德组。曲安奈德组大体标本观察软骨损伤分级优于对照组(P<0.05);电镜超微结构观察曲安奈德组在细胞形态、细胞质改变等方面优于对照组。对照组与生理盐水组比较差异无统计学意义(P>0.05);两组电镜观察细胞超微结构的改变相似。结论关节腔内注射徐长卿丹皮酚能推进损伤软骨的自我修复,延缓兔膝关节炎模型的关节软骨退变进程;激素类药物关节腔注射改善关节软骨退变作用不明显。Objective Through ultrastructural observation of Cynanchum paniculatum paeonol injection to articular cavity pathology of rabbit knee osteoarthritis model,whether paeonol has cartilage degeneration of osteoarthritis joint repair effect.Methods 40 male rabbits were randomly divided into normal group,control group,saline group,triamcinolone acetonide group,Cynanchum paniculatum paeonol group.Randomly selected 8 rats as normal group,the remaining 32 by rabbit knee joint anterior cruciate ligament transection（ACLT） and the medial meniscus resection before 1 / 3 production of osteoarthritis（OA） model method of molding,model succeed 4 weeks later.The physiological saline group,Maria Nade group,Cynanchum paniculatum paeonol groups were intra-articular injection of 0.9% saline,Maria Nade,paeonol each 0.2ml,2 times / week,a total of 10 times.Treatment after 1 weeks of exposure,knee joint cavity,gross observation and pathological changes of articular cartilage were observed under the electron microscope;the ultrastructure changes.Results Paeonol group general sample observation of cartilage damage grade is better than that of triamcinolone acetonide group（P0.05）;pathological changes of ultrastructure was lighter than that of triamcinolone acetonide group.Triamcinolone acetonide group gross observation of cartilage damage grade is better than that of control group（P0.05）;ultrastructure of triamcinolone acetonide group than in the cell shape,cytoplasmic changes the control group.The control group and the saline group had no significant difference（P 0.05）;the two group to observe the cell ultrastructure changes similar to.Conclusion intra-articular injection of paeonol can promote cartilage repair damage,delay of rabbit osteoarthritis model of articular cartilage degeneration process;steroids intra-articular injection of articular cartilage degeneration effect is not obvious improvement.
