锁骨钩钢板治疗肩锁关节脱位及锁骨远端骨折的临床观察

摘　　要：目的对锁骨钩钢板治疗肩锁关节脱位及锁骨远端骨折的临床观察。方法对2008-01-2010-01收治的锁骨远端骨折及TossyШ肩锁关节脱位的32例患者用锁骨钩钢板固定,用患者的健侧对比,用Constant and Murley方法评价肩关节的功能。结果对32例患者进行随访,随访5～12个月,平均随访8个月,内固定均无松动及断裂,骨折均愈合,肩锁关节脱位得到纠正。用患者的患侧与健侧对比,患侧的评分(76.4±8.5),健侧的评分99.5±0.5,(P<0.05)有统计学意义。结论锁骨钩钢板治疗肩锁关节脱位及锁骨远端骨折是非常实用的方法,能使脱位复位,韧带修复后维持原位。能保证锁骨远端骨折牢固固定,促进骨折愈合。该手术多种原因会影响关节功能,内固定物早期取出后,关节功能明显恢复。建议提高手术技巧,术后尽早取出。防止并发症的出现。Objective TO study the effect of clavicular hook plate in the treatment of acromioclavicular joint dislocations and fracture of distal clavicle.Methods rom january 2008 to january 2010,32 patients with fresh Tossy type III acromioclavicular joint dislocations and patients with fresh distal clavicle fractures were treated by clavicular hook plate.Contrast with the health side,The shoulder functions were evaluated according to constant and murley method.Results 32 patients were followed up from 5 to 12 months,（averaging 8 months）.there were no lossening or breaking.of interal fixations.the X-ray films showed that all wounds have been healed and acromioclavicular joints was achieved.Contrast with the health side,the scores of wound shoulder joint function were 76.4±8.5,the scores of the health side were 99.5±0.5,（P0.05）.Conclusion Clavicular hook plate for treatment of acromioclavicular joint dislocation and distal clavicular fracture is a very practical method,can make the dislocation,ligament repair maintenance after in situ.To ensure the distal clavicle fracture fixed,in promoting fracture healing.The operation for a variety of reasons to affect the function of joint,internal fixation after early removal,joint function was restored in 3.Suggestions for improving the operation skills,postoperative early removal.Prevention of complications.
