心电图“镜面”导联ST段改变对下壁心肌梗死相关血管的判定价值

摘　　要：目的:研究急性下壁心肌梗死心电图"镜面"导联ST段改变对梗死相关血管(IRA)的判定价值。方法:分析69例首次发生急性下壁心肌梗死患者体表心电图某些特征性改变,对比患者入院时冠状动脉造影资料。结果:急性下壁心肌梗死伴下列1项或以上者对预测IRA为右冠状动脉近段有较高的敏感性和特异性:①STⅢ↑>STⅡ↑;②STaVL↓>STⅠ↓;③STV1/V2≥0和STV2/V3<0,④STV3↓/STⅢ↑<0.5,其中急性下壁心肌梗死伴②时预测IRA为右冠状动脉近段的敏感性和特异性最高。结论:心电图"镜面"导联ST改变对下壁心肌梗死IRA判定有一定价值。Objective：Through the analysis of some characteristic changes of the acute inferior wall myocardial infarction（AIMI） in electrocardiogram,we compared them with the outcome of coronary angiography and researched the relationship between ST-segment change of mirror lead in AIMI and coronary infarct related artery（IRA）. Method：The clinical electrocardiogram change and the result of coronary angiography of 69 patients with first AIMI were analyzed in study. Result：AIMI with the following paragraphs 1 or more had the RCA proximal as the IRA： ①AIMI with STⅢ↑STⅡ↑;②AIMI with STaVL↓STⅠ↓;③AIMI with STV1/V2≥0 and STV2/V30;④AIMI with STV3↓/STⅢ↑0.5.AIMI with the paragraphs ② in predicting the IRA located the RCA proximal had the highest sensitivity and specificity. Conclusion：The AIMI with STV4-V6↓ is related with coronary multivessel disease.
