低频超声辐照载紫杉醇微泡干预低龄兔血管再狭窄的实验研究

摘　　要：目的:探讨不同超声参数条件辐照载紫杉醇微泡对低龄兔髂动脉血管再狭窄模型的干预作用及安全性。方法:新西兰大白兔25只,日龄(60±15)d,随机分为正常组、手术组和干预组,手术组和干预组经球囊扩张术建立髂动脉狭窄模型,干预组术后3周采用3种不同超声参数条件辐照载紫杉醇微泡对狭窄段血管进行干预,干预后1周观察各组管腔面积及内-中膜面积变化情况。结果:手术组管腔面积明显减小,内-中膜面积明显增加,狭窄率高达(80.22±6.70)%,与正常组相比较具有显著差异(P<0.05);干预组管腔不规则,内膜部分增厚,干预后各组管腔面积较治疗前扩大(P<0.01),内-中膜面积减小(P<0.01),管腔狭窄率降低(P<0.01),但较正常组仍有狭窄(P<0.05);在辐照参数为1MHz、0.5 W/cm2的条件下干预效果最为明显,管腔面积较其余干预组增大(P<0.05),内-中膜面积最小(P<0.05),狭窄率仅为(16.17±10.45)%。结论:在各低频超声辐照参数条件下辐照紫杉醇微泡对低龄兔血管结构重建均有一定程度的抑制,在辐照参数为1 MHz、0.5 W/cm2条件下干预效果最为明显。Objective：To explore the parameters of the low-frequency ultrasound on intervening vascular reconstruction in young rabbits by irradiating with different low-frequency ultrasound parameters. Method：The 25 young New Zealand rabbits,aged（60±15）days,were randomly divided into normal group,operation group and intervention group.We established the iliac artery stenosis model by balloon dilatation of the iliac arteries in operation group and intervention group,and intervened the paclitaxel-carrying microbubbles with different intensities and frequencies ultrasonic irradiation 3 weeks later in intervention group.The lumen area and tunica intima area were measured after 1 week of the intervention. Result：In the operation group,the tunica intima area was magnified compared with normal group,when the stenosis rate was（80.22±6.70）%,with cells disarranged,which was different from that in the nomal group（P0.05）.In the intervention subgroups,part of the intima was thickened,with membrane cells arranged.The lumen area was enlarged in all the intervention subgroups（P0.01）,when the tunica intima area was decreased（P0.01） and the stenosis rate was reduced（P0.01）,but was still narrower than that in the nomal group（P0.05）.The effect was obvious in the intervention group by irradiation conditions with the parameters as 1 MHz,0.5 W/cm2,in which the lumen area was larger than any other groups,and with the minimum tunica intima area（P0.05）,and the stenosis rate was（16.17±10.45）%. Conclusion：The vascular reconstruction can be intervented by different parameters of low-frequency ultrasound,especially in condition with the parameters as 1 MHz,0.5 W/cm2.
