摘 要:目的探讨利多卡因对肠缺血一再灌注(ischemia/reperfusion,I/R)损伤幼鼠肠组织表达肿瘤坏死因子a(TNF-a)和细胞间粘附分子1(ICAM-1)的影响。方法将48只4周龄SD大鼠随机分成三组,每组16只:假手术组(Sham组),仅暴露腹腔脏器,不作任何干预;缺血一再灌注组(I/R组)和利多卡因干预组(Lid组),阻断肠系膜上动脉造成缺血60min,I/R组再灌注前经股静脉注射生理盐水0.5ml,Lid组再灌注前经股静脉注射利多卡因2mg/kg。于再灌注后30、60、90、120rain每组动物各处死4只取小肠组织进行免疫组织化学染色,检测肠组织TNF-c~、ICAM-I的表达,结果进行组间单因素方差分析。结果正常肠组织中TNF-~和ICAM-1呈低水平表达,而从再灌注30min开始在I/R组和Lid组肠组织中TNFHn呈持续的高表达状态,但这两组中ICAM-1的表达呈缓慢上升趋势;通过测定TNF_a、ICAM-1表达的光密度值(0D值)在组间比较,I/R组和Lid组各时间点测得值均高于Sham组的相应测得值,差异有统计学意义(Pd0.05),其中两个指标均以I/R组的测得值为最高,差异有显著性(P〈0.05),仅ICAM-1于30、60min表达的OD值在I/R组和Lid组间比较差异无统计学意义(P〉0.05)。结论幼鼠再灌注肠组织中TNF-a和1CAM-1的表达增加,利多卡因通过下调肠组织中TNF-a、ICAM-1的表达而对生长发育期动物肠组织I/R损伤有保护作用。Objective To explore the effects of lidocaine on the expressions of tumor necrosis factor-a (TNF-a) and intercellular adhesion molecule-1 (ICAM-1) in intestines subjected to ischemia/ reperfusion (I/R) injury. Methods In this study, 48 Sprague-Dawley (SD) rats aged 4 weeks old were randomly divided into 3 groups according to their treatment. In the I/R group, the abdominal cavity of the rats was opened, and the intestinal I/R injury was induced by 60 minutes" superior mesen- teric artery occlusion followed by reperfusion for 30 min, 60 min, 90 min and 120 min. In the lidocaine treatment group, the rats were subjected to intestinal I/R injury, and administered with Lidocaine by femoral vein injection before reperfusion. The sham operated rats were only performed laparotomy. The intestines were harvested for immunohistochemical study to explore the expressions of TNF-a and ICAM-1. Results The sham operated rats had low TNF-a and ICAM-1 expressions. After I/R injury, the expressions of TNF-a and ICAM-1 in intestine were significantly elevated, but Lidocaine treatment inhibited the I/R induced up-regulation of TNF-a and ICAM-1 (all P%0. 05). The intestinal expres- sion of ICAM-1 remained unchanged in 30 min and 60 min after I/R (all P^0. 05). Conclusions Lido- caine down-regulates the expressions of TNF-a and ICAM-1 in intestine after ischemia/reperfusion in- iury in young rats.
