肠镜下凝血酶喷洒治疗过敏性紫癜合并顽固性下消化道出血疗效观察

摘　　要：目的观察肠镜下凝血酶喷洒治疗儿童过敏性紫癜(HSP)合并顽固性下消化道出血的疗效。方法选择2010年9月至2012年4月收治的25例HSP合并顽固性下消化道出血患儿,将其分为治疗组10例和对照组15例,均给予常规治疗,治疗组另加肠镜下凝血酶喷洒治疗。比较两组患儿治疗效果。结果治疗组患儿腹痛、血便等消化道症状改善显著优于对照组,大便隐血转阴较对照组快,激素用量较对照组减少,住院时间也短于对照组,差异均有统计学意义(P均<0.01)。结论肠镜下凝血酶喷洒治疗儿童HSP合并顽固性下消化道出血疗效明确,可减少激素使用量及住院时间。Objective To observe the curative effect of thrombin spray under colonoscopy on refractory lower gastroin- testinal bleeding in Henoch-Schfnlein purpura （HSP） in children. Methods Twenty-five children with HSP complicated with refractory lower gastrointestinal bleeding admitted to the department of nephrology from Sep. 2010 to Apr. 2012 were divided into the treatment group （11=10） and the control group （n=15）. Children in the treatment group received thrombin spray under colonoscopy in addition to the conventional therapy, and children in the control group received the conventional therapy only. The therapeutic effects were compared between two groups. Results Compared with the control group, the therapeutic effects of children in the treatment group were greater in that the gastrointestinal symptoms such as stomachache and bloody stools were relieved significantly, the time taken for occult blood test to be negative was shorter, the average steroid dosage was less, and the hospitalized stays were shorter （all P〈0.01）. Conclusions Thrombin spray under eolonoseopy for HSP with refractory lower gastrointestinal bleeding in children is definitely effective and it shortens the hospitalized stays and also decreases the total dose of steroid.
