摘  要:目的观察局部注射凝血酶联合无水乙醇对兔正常肝脏的消融作用。方法选取6只新西兰大白兔,麻醉后手术暴露肝脏,向肝左叶注入250IU凝血酶,并于给药前、给药后即刻行CEUS,以视觉观察并测量造影峰值强度(PI);在左叶凝血酶注射区和右叶相应位置(用于对照)各注入0.1ml无水乙醇,48h后测量左、右肝叶坏死直径并对其进行病理组织学检查。结果给药前兔肝左叶实质造影增强显著且均匀,PI为(-50.38±4.12)dB,给药后靶区造影剂充盈明显下降,PI降至(-59.77±3.28)dB(P=0.003);而给药前、后肝脏右叶造影剂充盈强度未见明显变化。给药48h后肝左叶坏死平均直径为(1.73±0.54)cm,右叶为(0.62±0.23)cm(P=0.001)。结论局部注射凝血酶可以部分阻断兔正常肝脏血供,显著增强无水乙醇对兔正常肝脏组织的消融作用。Objective To explore the effect of ethanol ablation (EA) combined with ultrasound-guided thrombin injection for rabbit liver. Methods The livers of 6 healthy New Zealand rabbits were surgically exposed. The left lobe was subjected to the treatment of thrombin (250 IU) combined with EA, while the right lobe was treated with EA only (served as the controls). CEUS was performed before and immediately after thromloin injection to estimate the blood perfusion of liver. Then the treated livers were collected 48 h after treatment to measure the mean necrotic diameters, and then were observed pathologically. Results CEUS demonstrated that the perfusion of left lobe was arrested after injection of thrombin, and the peak intensity (PI) dropped from (--50.38±4. 12)dB to (--59.77±3.28)dB (P=0. 003), while the right lobe, per- fusion remained homogeneous as before. The mean diameter of the necrotic area in left lobe was (1.73±0.54)cm, while in right lobe was (0.62±0.23)cm (P=0. 001). Conclusion Ultrasound-guided thrombin injection can partially block blood perfusion of rabbit liver and greatly promote the effect of ethanol ablation for rabbit liver.
