摘 要:目的 :研究新型Ⅲ类抗心律失常药物MS 5 5 1,即 1,3二甲基 6 [2 (N 2羟乙基 3 4硝苯丙氨基 )乙氨基 ] 2 ,4(1H ,3H) 盐酸嘧啶二酮 (MS) ,对麻醉犬心肌的电生理作用和对缺血性室性心律失常的防治效果。方法 :测定用药(MS首剂 0 5mg/kg于 5min内静推 ,维持量 0 5mg/kg静滴 30min)前后正常犬心肌房室不应期 ,并观察MS[首剂 0 3mg/kg于 5min内静推 ,维持量 0 5mg/ (kg·min)静滴 30min]对程序刺激诱发梗塞后犬缺血性室性心律失常的预防作用。结果 :MS显著延长麻醉犬心房和心室有效不应期 (ms) (12 8.6± 6 7vs 15 5 7± 4 8和 12 2 9± 7 5vs 143 6± 5 0 ,P均 <0 .0 1) ,以及QTc间期 (10 4± 0 4vs 12 0± 0 6 )。 7只梗塞后犬中有 5只经程序刺激诱发出室颤 ,用MS后无 1例再发室颤。未见致心律失常作用。结论 :MS可考虑作为防治缺血性室性心律失常的选择用药之一 ,对预防室颤具有一定意义。Aim: MS-551,1,3-dimethyl-6-{2-[N-(2-hydroxyethyl)-3-(4-nitrophenyl)propyl amino]ethylamino}-2,4(1H,3H)-pyrimidinedione hydrochloride (MS),is a newly synthesized class Ⅲ antiarrhythmic agent. We observed the electrophysiological effects of MS and its effects on prevention of ischaemic ventricular arrhythmias.Methods: Eight dogs were studied by the method of self-paired comparison.After anaesthetized,the atrial effective refractory period(AERP) and the ventricular effective refractory period(VERP) were examined before and after injection of MS (0.5 mg/kg/5 min+0.5 mg/ kg/30 min iv) by using cardiac catheters.Then, the prevention effects of MS (0.3 mg/kg/ 5 min+0.05 mg/kg/min ×30min iv) on ischaemic ventricular arrhythmias were observed in postinfarction dogs with the use of programmed electrical stimulation(PES) through epicardiac electrodes. Results:MS increased markedly both the AERP(128.6±6.7 ms vs. 155.7±4.8 ms. P <0.01) and VERP(122.9±7.5 ms vs. 143.6±5.0 ms, P < 0.01 ) and prolonged QTc (10.4±0.4 vs. 12.0±0.6) noticeably. The ventricular fibrillations (VF) were induced by PES successfully in five of seven postinfarction dogs. After injection of MS, however,no VF was inducible in any dog. No proarrhythmic effects (such as tosades de pointes) were observed. Conclusion: MS might be used intravenously to prevent the ischaemic ventricular arrhythmias, and that MS has the benefit effect on the prevention of VF.
