摘  要:目的:探讨核酸恒温扩增-免疫层析检测方法对布鲁菌病(布病)诊断的应用价值,为核酸检测标准化推广应用提供实验室参考依据。方法:分别以全血和血清为样本提取布鲁菌核酸,应用恒温扩增技术进行核酸扩增,以免疫层析检测装置直接判读核酸扩增结果;免疫学检测应用试管凝集试验(SAT);数据处理应用配对校正χ2检验。结果:29例就诊者全血核酸扩增阳性率为55.2%,血清核酸扩增阳性率为23.1%,二者比较差异有统计学意义(P<0.05);以全血为核酸提取样本,190例不同病程SAT诊断为阳性和阴性的就诊者核酸检测结果与SAT检测结果符合率为57.9%;93例首诊SAT诊断为阳性和阴性的急性早期就诊者核酸检测结果与SAT检测结果符合率为63.4%。结论:核酸恒温扩增-免疫层析检测方法作为一项实验室辅助诊断和治疗的参考依据,对急性早期的布病患者具有较高的临床应用价值,在鉴别诊断布病患者和非患者方面无参考价值。Objective To investigate the application value of nucleic acid isothermal amplification-immunochromatographic test in the diagnosis of brucellosis and to provide experimental basis for clinical application of nucleic acid test.Methods The brucella nucleic acid was extracted from blood and serum samples,and isothermal amplification technique was applied to amplify nucleic acid and immunochromatographic test device was used to interpret the results of nucleic acid amplification directly.Tube agglutination test(SAT) was used as immunological tests.The pairing Chi-square test was performed in the data analyses.Results The brucella nucleic acid was extracted from the whole blood and serum of 29 cases,the amplification rate of whole blood nucleic acid was 55.2%,the amplification rate of serum nucleic acid was 23.1%,and there was significant difference(P0.05).The nucleic acid was extracted from the whole blood of 190 cases in different course,using SAT diagnostic technique,the cases were divided into positive and negative treatment populations,to these people,the agreement rate of the nucleic acid test results and SAT test results was 57.9%.93 cases who saw the doctor at the first time were divided into positive and negative treatment populations with SAT diagnostic technique,to these people,the agreement rate of the nucleic acid test results and SAT test results was 63.4%.Conclusion As a reference of laboratory-assisted diagnosis and treatment,nucleic acid isothermal amplification-immunochromatographic test has higher clinical application value for acute early brucellosis cases,but for differential diagnosis has no reference value for brucellosis patients and non-brucellosis patients.
