摘  要:目的探讨实时三维超声在评价冠心病心肌梗塞患者左室壁运动同步性的价值。方法对25名左室心肌梗死患者和30名健康志愿者进行常规超声心动图及实时三维超声心动图(RT-3DE)检查,测量左室功能参数及分析17节段时间-容积曲线,计算达到最小收缩容积时间(Tmsv)的标准差(Tmsv16-SD、Tmsv12-SD、Tmsv6-SD)和最大差值(Tmsv16-Dif、Tmsv12-Dif、Tmsv6-Dif),以Tmsv16-SD%大于正常对照组上限99%为左室收缩不同步指数(systolicdyssynchrony index,SDI)。结果急性心肌梗死(AMI)组各收缩同步性参数均显著大于正常对照组(P<0.01);以SDI超过正常对照组单侧99%上限界值计算SDI为2.6%。结论实时三维超声心动图能够有效的评价心肌梗塞患者左心室收缩同步性。Objective To evaluate the left ventricular systolic synchrony using real - time three - dimensional echocardiography (RT - 3DE) on patients with acute myocardial infarction (AMI). Methods Conventional e- chocardiography and RT- 3DE were performaned on 25 patients with AMI and 30 normal subjects (control group). The standard deviation (Tmsv- SD) and the maximal difference (Tmsv- Dif) of Tmsv were calculated. The systolic dyssynchronyindex ( SDI ) was defined as the standard deviation of time - to - minimal systolic volume of the16 Lvsegments expressed in percentage of R - R duration. Results Parameters index ( including Tmsv16 - SD, Tmsv12 - SD, Tmsv6 - SD, Tmsv16 - Dif, Tmsv12 - Dif, Tmsv6 - Dif, Tmsv16 - SD%, Tmsv12 - SD%, Trnsv6 - SD% ,Tmsv16 - Dif%, Tmsv12 - Dif%, Tmsv6 - Dif% ) was significantly higher in patients than that in normal subject. Conclusion Left ventricular dyssynchrony exist in myocardial infraction patients. RT- 3DE is an avaliable technique for left ventricular synchronicity quantification.
