摘  要:目的调查昆明医科大学第一附属医院2011年嗜麦芽窄食单胞菌的耐药性,以指导临床合理使用抗菌药和预防院内感染。方法回顾性分析昆明医科大学第一附属医院2011年检出的嗜麦芽窄食单胞菌的耐药情况,并与2010年进行比较。结果 2011年在昆明医科大学第一附属医院分离到3878株细菌,其中非发酵菌729株,嗜麦芽窄食单胞菌为113株,占全部分离菌株的2.9%,占非发酵菌株的比例为15.5%,大部分来自呼吸道标本,呼吸道标本检出71株(占62.8%),血液标本27株(占23.8%)。2011年昆明医科大学第一附属医院检出的嗜麦芽窄食单胞菌对头孢他啶的耐药率为62.8%,与2010年相比,其对头胞哌酮舒巴坦和复方磺胺甲口恶唑的耐药率分别由11.2%上升至45.1%和由32.9%上升至45.1%,对左氧氟沙星的耐药率由17.9%下降至13.3%,对米诺环素的耐药率变化不明显。结论昆明医科大学第一附属医院嗜麦芽窄食单胞菌对常用抗菌药的耐药率仍较明显,尤其是对头胞哌酮舒巴坦的耐药率出现了显著升高。应加强细菌耐药监测,指导临床合理使用抗菌药物,同时要加强感染控制措施,预防和控制耐药菌株的院内流行。Objective To investigate the antimicrobial resistance of Stenotrophomonas maltophilia isolated from patients in the First Affiliated Hospital of Kunming Medical University during 2011. Methods Clinical isolates of S. maltophilia were collected from our hospital in 2011. The results of drug susceptibility test were compared with 2010 situation. Results Of the 3878 clinical strains collected, including 729 strains of non-fermenting bacteria, S. maltophilia was 113 (2.9% of all the isolated strains, 15.5% of non-fermenting bacteria ) , and 71 strains ( 62.8% ) were isolated from respiratory tract, 27 strains ( 23.8% ) were from blood. The percentage of S. maltophilia resistance to Cerftazidime was 62.8%. Compared with the data of year 2010, he percentage of S. maltophilia resistance to cefo-perazone-sullbactam increased from 11.2% 45.1% ,to cotrimoxazole increased from 32.9% to 45.1% ,to levofloxacin decreased from 17.9% to 13.3%, and to minocycline was not different. Conclusion The antimicrobial resistance of S. maltophilia is still a serious problem in our hospital. Especially, the percentage of S. rnaltophilia resistance to cefoperazone-sullbaetam is obviously increased. The surveillance of antimierebial resistance is very important for guiding rational antimicrobial therapy, and infection control should be strengthened to prevent the prevalence of the drug-resistant bacteria.
