多次输血患者血小板抗体检测及临床相关研究

摘　　要：目的:观察多次输血血液病患者血小板抗体与血小板输注效果。方法:采用简易致敏血小板血清学技术(SPE-SA)分别比较多次输血患者(实验组)和无输血史的住院患者(对照组)的血小板抗体阳性率,对血小板抗体阳性患者进行血小板交叉配型,计算1h和24h血小板增高指数(CCI值),对配合型血小板输注效果进行比较分析。结果:血小板无效输注率为32.6%;实验组和对照组中的血小板抗体阳性率分别为42.1%和14.2%,组间差异有统计学意义(P<0.05),实验组中抗体阳性率与输注次数呈正相关(r2=0.853),血小板抗体阳性患者输注随机血小板45次后CCI值和输注配型血小板30次后CCI值比较,有显著性差异。结论:血小板抗体检测及抗体类型的确认,配合性地输注血小板,有利于提高血小板输注的有效性。Objective： To observae the effect of platelet transfusion and blood platelet antibody causing by multiple blood transfusion. Methods： Platelet antibody positive rate was compared by using simplified sensitized platelet serology assay （SEPSA） in patients with repeated blood transfusions （experimental group） and without a history of patients （control group）then measures of platelet cross matching, calculation of 1 h and 24h thrombocytosis index （CCI value）were taken on platelet antibody positive patients so that the matched type platelet effect of transfusion can be analysed. Results：The invalid infusion rate of platelet is 32. 6%. The platelet antibody positive rates, with difference between groups was statistically significant （P〈0.05）, were 42. 1% and 14. 2% in experimental group and control group respectively. The experimental group antibody positive rate was positively correlated with blood infusion times （r^2 =0. 853）. There was significant difference between CCI of patients platelet antibody positive infused with random platelet for 45 times and patients infused with matched platelet for 30 times. Conclusion：Platelet antibody test and antibody type of confirmation, cooperated with platelet cross-matching transfusion, which is beneficial to improve the effectiveness of platelet transfusion.
