重度子痫前期心脏形态及功能的研究

摘　　要：目的:探讨重度子痫前期患者心脏形态及功能的变化。方法:早发型重度子痫前期46例,晚发型重度子痫前期62例,正常妊娠86例,比较彩色多普勒心脏超声结果中各指标的差异来评估心脏形态及功能的变化。结果:与正常妊娠组相比,晚发型重度子痫前期组左心各径线明显增大,差异有统计学意义(P<0.05);收缩期指标差异无统计学意义(P>0.05),但4例LVEF<55%;舒张期指标显著降低,差异有统计学意义(P<0.01);心包积液发生率显著增高,差异有统计学意义(P<0.05)。早发型重度子痫前期比晚发型左房增大更显著,差异有统计学意义(P<0.01)。结论:重度子痫前期患者心脏形态改变以左房室腔增大、室壁及室间隔增厚、心包积液为主,伴左室舒张功能的损害,个别病例收缩功能轻度受损。早发型重度子痫前期心血管损害程度超过晚发型。Objective： To explore the changes of cardiac morphology and function of patients with severe preeclampsia. Methods： Forty - six patients with early - onset severe preeclampsia, 62 patients with late - onset severe preeclampsia, and 86 patients of normal preg- nancy were selected, the changes of cardiac morphology and function were evaluated by comparing the differences of various indexes of color Doppler uhrasonography. Results： Compared with normal pregnancy group, the diameters of left heart in late - onset severe preeclampsia group increased, there were statistically significant differences （ P 〈 O. 05 ） ; there was no statistically significant difference in the indexes during systolic period （P 〉 0. 05 ）, LVEF of four patients was less than 55% ; the indexes during diastolic period decreased significantly. there were statistically significant differences （P 〈 0. 01 ） ; the incidence rate of hydroperieardium increased significantly, there was statisti- cally significant difference （ P 〈 0. 05 ） . The size of left atrium in early - onset severe preeclampsia group was statistically significantly larger than that in late - onset severe preeclampsia group （P 〈 0. 01 ） . Conclusion： The main changes of cardiac morphology in patients with se- vere preeclampsia included left atrial enlargement, left ventricular enlargement, ventricular wall thickening, interventricular septum thicken- ing, and hydropericardium, combining with injury of left ventrieular diastolic function, individual patients were found with mild injury of con- traction function. The degree of cardiovascular injury in patients with early - onset severe preeclampsia was severer than that with late - onset severe preeclampsia.
