社区基本公共卫生服务效果与预防保健机构技术支持的关系研究

摘　　要：目的分析预防保健机构技术支持与社区基本公共卫生服务效果的关系。方法 2010年6-9月采用现场调查的方法,调查青岛市所辖的区市疾病预防控制(疾控)机构、妇幼保健机构、社区卫生服务机构的工作人员和负责人。计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果在合作关系上,认为目前与社区机构合作关系良好疾控部门有34.7%,妇幼机构有31.9%,社区机构人员中有16.7%,比较差异有统计学意义(χ2=10.200,P<0.05)。仅有33.3%疾病预防控制机构和25.0%妇幼机构将项目技术指导纳入2010年工作计划;疾控部门中72.2%的调查对象认为开展技术指导时不顺利,妇幼机构中则有68.1%;社区卫生服务机构有53.8%的调查对象认为需要上级部门协调,比较差异有统计意义(χ2=8.107,P<0.05)。有58.3%通过卫生行政部门协调建立了基本公共卫生服务项目的信息报告制度;90.3%的疾控部门工作人员,91.7%的妇幼机构工作人员和87.9%的社区工作人员认为信息沟通机制不健全,获得社区项目进展信息有障碍。所调查的12家疾病预防控制机构与妇幼保健机构共开展项目技术指导165次,平均(13.75±9.46)次;指导社区卫生服务机构312次,平均指导(26.00±16.79)次,差异有统计学意义(P<0.05)。结论预防保健机构与社区卫生服务机构间的合作关系欠佳,两者间的技术指导工作开展多不顺利,信息沟通也多存在障碍。Objective To analyze the relationship between insitutions of preventive health care mad the effectiveness of basic public health services in community. Methods On - the - spot investigation was carried out anaong staff and leaders in centers for disease control and prevention from June to September 2010, maternal and child healthcare institutions and community health services in Qingdao city. The t - test was used for measurement data and ~ test for count data, the result of P 〈 0.05 was considered to be statistically different. Results For cooperation, 34.7% of the staff＂ in centers for disease prevention and control, 31.9% in maternal and child bealtheare institutions and 16.7% in community health services thought they had a good coopration with community health centers. The difference was statiseitally significant （χ2 = 10. 200,P 〈0.05）. Only 33.3% of centers for disease prevention and control and 5.0% of maternity and child care institu- tions included technical guidance for project in 2010 work plan. 72.2% of the staff in centers for disease prevention and control, 68.1% in maternal and child healthcare considered their application of technical guaidance unsmoothly; 53.8% in community health services said that coordination from higher authorities were needed. The difference was statiseitally sig- nificant （χ2 =8. 107,P 〈0.05）. 58.3% of the institutions established their information reporting system in basic public health services with the eorrdination of health administrative departments. 90.3% of the staff in centers for disease preven- tion and control, 91.7% in maternal and child healtheare institutions and 87.9% in community health services held that the information communication mechanism was not perfect, which made them difficult to obtain the information on communi- ty projects. A total of 165 times of project technical guidance was carried out by centers for disease prevention and control and maternal and child healthcare institutions with an average of （13.75 ±9.46） times;and
