胺碘酮与厄贝沙坦联合治疗阵发性房颤临床研究

摘　　要：目的观察口服胺碘酮联合应用血管紧张素Ⅱ受体拮抗剂(ARB)厄贝沙坦在阵发性心房颤动复律后维持窦律的疗效及对左心房功能的影响。方法将146例阵发性房颤患者分为单纯口服胺碘酮组(对照组,72例)和胺碘酮加厄贝沙坦组(治疗组,74例,厄贝沙坦150 mg/d),疗效观察12个月。结果共146例患者完成治疗,随访12个月,治疗1、3个月后两组左心房内径大小无明显变化,6个月后治疗组左房大小明显小于对照组(P<0.05),治疗组6个月后窦性心律维持率治疗组明显高于对照组,差异有统计学意义(P<0.05)。结论胺碘酮与厄贝沙坦联合治疗阵发性房颤维持窦性心律的疗效优于单用胺碘酮,并能延缓左心房扩大。Objective To observe the effect of combination of amiodarone and Renin-Angiotensin Sys- tem （RAS） inhibitors irbesartan maintenance of sinus rhythm after cardioversion in patients with paroxysmal at- rial fibrillation and its influence on left atrial function. Methods 146 patients with paroxysmal AF were ran- domly divided into two groups ： amiodarone group （n = 72 ） and amiodarone plus Irbesartan group （n = 74）. The duration of observation was up to 12 months. Results After the treatment and 12 month follow-up, There was no statistically significant difference in the left atrial diameter（LAD） between Group I and Group II in the 1 st and the 3rd month. But there was significant difference （P 〈 0. 05 ） in maintenance rate of sinus rhythm and left atrial diameter in the 6th and the 12th month. Conclusion The combination of amiodarone and irbesartan is more effective than amiodarone alone for sinus rhythm maintenance, and can restrain the enlargement of the left atrium.
