EDTA依赖性假性血小板减少症不同计数模式结果分析

摘　　要：目的以手工草酸铵法为质量评价,分析EDTA依赖性假性血小板减少症(PTCP)样本在全自动血细胞分析仪全血模式、稀释模式下的结果变化,为实验室提供方法依据。方法结合显微镜复检规则,应用全血细胞分析法、仪器稀释法、手工草酸铵法同时对PTCP样本进行检测。结果患者标本采集第5分钟时血小板检测值基本一致;应用全血模式分析,患者血小板检测结果在不同时间段差异较大;应用稀释模式分析第一种结果:患者血小板计数在不同时间段差异较大;第二种结果:患者血小板计数随着时间的延长相对稳定;第三种结果:患者血小板计数在60 min内发生分散-聚集-解离,使第60分钟时的检测结果与第5分钟时的检测结果基本一致。手工草酸铵法测定结果差异不大。全血模式计数结果 >CLIA’88总误差的1/3判断标准,结果不可接受。结论按照稀释模式进行PTCP复检,5 min完成计数是成本低廉并能保证检验质量的首选方法。Objective To check the platelet counting results in manual and dilution mode by hematology analyzer for EDTA-dependent pseudothrombocytopenia,provide basis to the methods of laboratory examinations.Methods Blood samples tested respctively by hematology analyzer in whole blood mode and dilution mode and manual performed with ammonium oxalate.Rechecked platelet counts by microscope.All testing regulations were compliance of Clinical Laboratory Improvement Amendments of 1988（CLIA＇88）.Results The platelet counts were nearly consistence at fifth minute.Platelet counts were more deviations at others time periods in whole blood mode and dilution mode.In dilution mode,the value of counting were relatively constant with the time prolonging.Because the platelet distributed,aggregated,dissociated in sixty minutes that led to nearly consistence of count value at the sixtieth and the fifth minute.Testing results were less changed performed with ammonium oxalate.Conclusions Complete the testing in five minutes in the dilution mode is preferred to rechecking platelet count.The cost is low and the result is reliable.
