双J管内引流配合ESWL治疗肾结石的临床观察

摘　　要：目的观察双J管预置内引流配合体外冲击波碎石(ESWL)治疗介于2.0cm～3.5cm的单纯肾结石的临床效果。方法收集2011年6月至2012年10月间收治的40例介于2.0cm～3.5cm的单纯肾结石,采用ESWL治疗,在治疗前预置使用双J管作内支架及引流。结果 40例肾结石治疗后1周后复查KUB,碎石效果不佳者做第二次ESWL,1次碎石者32例,2次碎石者6例;1个月结石排尽29例,2个月结石排尽6例,3个月结石排尽3例,2例碎石效果不佳仍有残留改行PCN取石。随诊3个月且根据碎石排石情况1～3个月内经膀胱镜拔除双J管,期间未有发现输尿管石街、严重尿路感染、严重血尿等并发症。24例出现程度不同的膀胱刺激症。结论双J管内引流配合ESWL治疗2.0cm～3.5cm的单纯肾结石,能够有效防治输尿管石街的并发症,效果满意,安全性好,可以适宜在基层医疗单位推广应用的治疗技术。Objective To observe the clinical effects on the double J tube drainage in coordinating ESWL for treatment of diameter in 2 cm to 3.5 cm simple renal stones. Methods From June 2011 to October 2012, 40 cases with simple renal stones were collected. The double J tube as internal stent and drainage was used before the treatment. Results The KUB was re-xaminaed in 40 cases 1 week after treatment of ESWL One time lithotripsy was in 32 cases. Twice lithotripsy was in 6 cases. The stones were removed completely which 1,2,3-month were in 29, 6 and 3 cases respectively. Two cases with residual stone underwent PCNL. Follwo-up time was in 3 months. According to the condition of removing stones, the double J tube was extracted by the cystoscopy within 1 to 3-month. No nreteral stone-street, severe urinary infection and severe hematuria were found. Twenty four cases occurred irritation sign of bladder in different degree. Conclusion With double J tube internal drainage in coordinating ESWL for treatment of simple renal stones in diameter 2 cm to 3.5 era, it would contribute to prevent and treat the complieation of ureteral stone-street and has a satisfactory result.
