摘　　要：目的:探讨人端粒酶RNA(hTERC)基因在宫颈病变脱落细胞中的表达及临床意义。方法:收集100例妇女宫颈脱落细胞标本,经病理学检查确诊,正常宫颈20例,CINI 11例,CINⅡ11例,CINⅢ8例,鳞癌30例,腺癌20例。应用荧光原位杂交(FISH)技术检测脱落细胞中hTERC基因的表达,比较其在不同病变细胞中的表达差异情况。结果:hTERC基因在正常宫颈脱落细胞、CINI、CINⅡ、CINⅢ及宫颈癌中的阳性表达率分别为0.00、18.18%、63.63%、100.00%、100.00%,CINⅡ组显著高于CINI组,CINⅢ、宫颈癌组显著高于CINI、CINⅡ组,差异有统计学意义(P<0.05)。随宫颈病变级别增高,hTERC基因扩增的阳性率显著增高,且异常细胞数和异常核型的复杂性均显著增多。结论:hTERC基因扩增表达水平随着宫颈病变级别增高而增加,能很好地预测宫颈病变的发展趋势,有望成为宫颈癌筛查、治疗及评估预后的重要指标之一。Objective： To explore expression and significance in cast-off cells of cervical lesions. Methosds. Cast-off cells of cervical lesions from 100 cases were collected, and confirmed by pathological a- nalysis. There were 20 cases with normal cervix, cases with CIN I ,11 cases with CIN II , 8 cases with CIN ~, 30 cases with squamous carcinoma, and 20 cases with adenocarcinoma. Expression of hTERC gene was detected and compared by fluorescence in situ hybridization. Results： The positive rate of hTERC gene was0.00~, 18.18~, 63. 63~ ,100. 00% ,100. 00% , in normal group, CINIgroup, CINI] group, CIN ]]] and adenoearcinoma group, respectively. The expression was significantly higher in CIN II group than CIN I group（P^0.05）. And it was significantly higher in adenocarcinoma group than CIN I group and CIN II group（P^0.05）. The higher the lesion level was, the higher the positive rate was, with more ab- normal cell counts and more cells in complicated abnormal karyotype. Conclusions. The higher the cervical lesion is, the higher expression level of hTERC gene is. hTERC is valuable in predict the progress of cer- vical lesion, and is an important index for screening, treatment and nr^n~i~ l,,o~;,~
