超声结合X线检查对银屑病性关节炎的诊断价值

摘　　要：目的探讨银屑病性关节炎的超声和X线影像学特征及二者在早期银屑病性关节炎诊断中的价值。方法对56例经临床及实验病理检查确诊为银屑病的患者进行超声检查和X线检查,对病变关节主要超声表现和X线检查结果采用SPSS 13.0统计学软件进行分析。结果超声对关节病变的检出率为8.5%,X线对关节病变的检出率为2.9%。在56例患者中,银屑病性关节炎患者中超声阳性为35例,X线阳性15例,超声联合X线40例。超声组与X线组对比差异有统计学意义(P=0.0003)。X线组与超声联合X线组对比差异有统计学意义(P<0.0001),超声与超声联合X线两组差异无统计学意义(P=0.422)。结论超声结合X线检查能提高银屑病性关节炎的检出率,超声检查是银屑病关节病变早期筛查的有效手段,可作为银屑病性关节炎软组织损伤首选的影像学检查方法。Objective To investigate imaging characteristics and early diagnosis value of ultrasound and X-ray, or both, on psoriatic arthritis. Methods Joint lesions of 56 patients with psoriatic arthritis were examined with the use of ultrasound or X-ray, and all findings of 56 patients with psoriatic arthritis were analyzed with SPSS 13.0 software. Results Detection rates of psoriatic arthritis of ultrasound and X-ray were 8.5% and 2.9%, respectively. Among 56 patients, positive of psoriatic arthritis of ultrasound, X-ray and ultrasound plus X-ray were 35 cases, 15 cases, 40 cases, respectively. There was significant difference between ultrasound and X-ray （P = 0.0003）, there was very statistical difference between X-ray and ultrasound plus X-ray （P 〈 0.0001）, but there was no significant difference between ultrasound and ultrasound plus X-ray （P = 0.422）. Conclusion Ultrasound plus X-ray can improve detection rate of psoriatic arthritis, and ultrasound is an effective tool, and preferred imaging of early screening of psoriasis joint lesions.
