人工鼻泪管植入术在慢性泪囊炎患者中的应用及护理

摘　　要：目的探讨人工鼻泪管植入术在慢性泪囊炎治疗中的应用价值及护理干预。方法将内蒙古医科大学附属医院收治的76例慢性泪囊炎患者随机分为观察组和对照组,每组各38例,对照组行传统泪囊鼻腔吻合术治疗和常规护理,观察组行人工鼻泪管植入术治疗并介入相应的护理干预,观察两组患者的临床治疗效果、术后并发症发生率和预后情况。结果观察组临床治疗总有效率为95.8%,对照组临床治疗总有效率为74.0%,两组比较差异有统计学意义(χ2=6.613,P=0.039);术后3个月随访,观察组泪道通畅率为89.6%,泪点出血率为2.1%,对照组泪道通畅率为76.0%,泪点出血率为18.0%,两组比较差异有统计学意义(χ2=6.262,P=0.044;χ2=7.617,P=0.028)。结论人工鼻泪管植入术治疗慢性泪囊炎能显著提高临床疗效与改善患者预后,相比传统泪囊鼻腔吻合术更具有临床应用价值。Objective To explore the value and nursing intervention of the implantation of artificial nasolacrimal duct in patients with chronic dacryocystitis. Methods 76 patients with chronic dacryocystitis were randomly divided into observation group and control group in the Affiliated Hospital of Inner Mongolia Medical, each group had 38 cases. The control group was given traditional dacryocystorhinostomy and conventional care, the observation group was given artificial nasolacrimal duct implantation and interventional care intervention, the clinical efficacy and prognosis of patients in two groups were observed. Results The total effective rate of the observation group was 95.8%, the total effec- tive rate of the control group was 74.0%, there was significant difference in the two groups of clinical efficacy x2=6.613, P =0.039）; after 3 months of follow-up, the lacrimal duct patency rate of the observation group was 89.6%, the tears of the bleeding rate was 2.1%, the lacrimal duct patency rate of the observation group was 76.0%, the tears of the bleeding rate was 18.0%, there were significant differences in the two groups of prognosis （X2=6.262, P = 0.044; X2= 7.617, P = 0.028）. Conclusion The implantation of artificial nasolacrimal duct can significantly improve the clinical efficacy and prognosis, it has important clinical value in the treatment of chronic dacryoeystitis.
