腹腔镜肝叶切除联合胆道镜治疗肝内外胆管结石的临床研究

摘　　要：目的:探讨腹腔镜左肝叶切除并联合胆道镜行胆总管探查取石的可行性、安全性及效果。方法:回顾分析2009年6月至2012年1月为45例左肝内胆管结石合并胆总管结石患者行腹腔镜左肝叶切除联合胆道镜胆总管切开取石术的临床资料。结果:45例手术均顺利完成。手术时间126～185 min,平均(145±47)min;术中出血量120～450 ml,平均(210±63)ml。术后4例发生胆漏。结论:腹腔镜左肝叶切除联合胆道镜行胆总管探查取石术是安全、有效的。Objective：To evaluate the feasibility,safety and efficacy of laparoscopic left hepatolobectomy combined with choledochoscopic common bile duct exploration.Methods：The clinical data of 45 patients who suffered from left intrahepatic bile duct calculi and choledocholith and underwent laparoscopic left hepatolobectomy combined with choledochoscopic choledocholithotomy from Jun.2009 to Jan.2012 were analyzed retrospectively.Results：All operations were conducted successfully.The operation duration was 126-185 min,mean（145±47） min.The intraoperative blood loss was 120-450 ml,mean（210±63） ml.Bile leakage occurred in 4 patients.Conclusions：Combined application of laparoscopic left hepatolobectomy and choledochoscopic common bile duct exploration is a safe and effective procedure for patients with intra-and extra-hepatic bile duct calculi.
